Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: In their later years, Americans of different racial and ethnic backgrounds are not in equally good-or equally poor-health. There is wide variation, but on average older Whites are healthier than older Blacks and tend to outlive them. But Whites tend to be in poorer health than Hispanics and Asian Americans. This volume documents the differentials and considers possible explanations. Selection processes play a role: selective migration, for instance, or selective survival to advanced ages. Health differentials originate early in life, possibly even before birth, and are affected by events and experiences throughout the life course. Differences in socioeconomic status, risk behavior, social relations, and health care all play a role. Separate chapters consider the contribution of such factors and the biopsychosocial mechanisms that link them to health. This volume provides the empirical evidence for the research agenda provided in the separate report of the Panel on Race, Ethnicity, and Health in Later Life.

Demographically the most important continent on the world, Asia's population, currently estimated to be 4.2 billion, is expected to increase to about 5.9 billion by 2050. Rapid declines in fertility, together with rising life expectancy, are altering the age structure of the population so that in 2050, for the first time in history, there will be roughly as many people in Asia over the age of 65 as under the age of 15. It is against this backdrop that the Division of Behavioral and Social Research at the U.S. National Institute on Aging (NIA) asked the National Research Council (NRC), through the Committee on Population, to undertake a project on advancing behavioral and social research on aging in Asia. Aging in Asia: Findings from New and Emerging Data Initiatives is a peer-reviewed collection of papers from China, India, Indonesia, Japan, and Thailand that were presented at two conferences organized in conjunction with the Chinese Academy of Sciences, Indian National Science Academy, Indonesian Academy of Sciences, and Science Council of Japan; the first conference was hosted by the Chinese Academy of Social Sciences in Beijing, and the second conference was hosted by the Indian National Science Academy in New Delhi. The papers in the volume highlight the contributions from new and emerging data initiatives in the region and cover subject areas such as economic growth, labor markets, and consumption; family roles and responsibilities; and labor markets and consumption.

Book excerpt: Access to health information is a crucial piece of the racial and ethnic health disparities puzzle among immigrants. Scholars investigating the role of social networks have shown that the number and types of networks among minorities and lower income groups differ. Few scholars have examined the use of social support in social networks to retrieve health information. No extant studies examine both availability of social support and social networks and health information seeking behaviors for Korean immigrants. This dissertation examines the influences of social support networks on health information seeking behaviors to increase understanding about the influences of social networks on such seeking by immigrants, especially the use of the Internet. The study investigates the effects of (a) demographic factors including age, gender, acculturation, (b) perceived social support, and (c) social networks on online health information seeking behaviors among Korean immigrants. Health information seeking behaviors are examined in four areas: different health information sources from social networks; topics of health information from social networks; online health information seeking behaviors. An intramethod approach utilizing both qualitative and quantitative approaches was used to provide both specificity and depth of response. An online survey was administered to 205 first generation immigrant Korean American men and women aged 18-49. Open-ended questions explored social support and social networks in health information seeking behavior for Korean Americans. Survey data expanded knowledge about the relationships between social support, social networks, and health information seeking behaviors, in particular online. Results from the qualitative data add to existing literature in health communication by demonstrating why social support and social networks are important for immigrants. Findings extend current research on health information seeking behaviors and social support to the Korean immigrant population. Results from the survey capture how the availability of social support and the size of social networks can influence health information seeking behaviors. Findings enhance the utility of SNT as a theory, as well as understanding of health communication for immigrants. SNT was applied for the first time in a health communication study of Korean immigrants' health information seeking behaviors. Results demonstrate how social networks function as sources of heath information for Korean Americans within the SNT framework. The utility of this theory includes health disparities and immigrant contexts. For health care practitioners and public policy makers, it provides empirical evidence about use of online health information and social network members as health information sources among Korean Americans. In-group online communities are effective channels for disseminating important health information targeting Korean Americans. This study presents suggestions for health care providers in offering culturally and linguistically appropriate care to Korean immigrants. Health care providers need to recognize Korean immigrants' expectations of health care services, unmet needs, and the reasons for complaints. Providers should also increase awareness about the US health care system among Korean immigrants and educate them to enhance their health care system literacy.

Daniel McMillin (PhD, Southern Illinois University), and Shienpei Chang (MA, California State University Bakersfield), this unique book examines how social networks contribute to a sense of well-being and a positive self-identity among older Americans and Taiwanese. Although social network analysis has grown increasingly important in the last several decades, few comparisons are available with Chinese and American samples; this is the first research project that compares a Western and an Asian culture using social network types. This research is also the first ever to use social network types to test hypotheses about values, reciprocity, social capital, and the health status of older adults. The data, gathered through systematic sampling in northeastern Oregon and central Taiwan, are first analyzed for the content of exchanges with network members. Then, the structure of the social network is determined by cluster analysis from which four network types are derived. This innovative, two-part procedure reveals a deeper understanding of the role social networks play in the quality of life among elderly in these two cultures. By comparing two very different cultures, the research reveals important details about the relative impact of broader social changes and social networks on the well-being of older adults. The two societies represent contrasting cultural sensibilities regarding the position and treatment of the aged. Yet, social changes in both countries have had a similar impact on older adults in some respects, but not in others. The data allow a determination of whether the inherent dissimilarities between a Western and an Asian culture, or the differences in the structure of each network type, can best account for the variation in exchange modalities and outcomes related to well-being and self-identity. A final chapter highlights possible future research in light of the theoretical and methodological implications of the findings. This book is a valuable resource for those in cultural anthropology, comparative sociology, gerontology, and Asian studies.

Book excerpt: Abstract: Epidemiological studies have shown that socially integrated older adults live longer and healthier lives. Attempts have been made through organizations like senior centers to enhance social integration among older adults, but little is known about the impact of these efforts. This study investigated the characteristics of social networks among older adults living in community settings and whether these characteristics influence older adults' intentions to participate in new social activities. A total of 126 face-to-face structured interviews were conducted with older adults between the ages of 65 and 85 living in an urban community in Columbus, OH. Addresses were randomly selected from all possible addresses in the area. Recruitment letters were sent by mail, with follow-up phone calls made when phone numbers were available. About 63.5% of the respondents were female, and 89.7% were White. The influence of social network characteristics, social support, companionship, and participation in productive activities on older adults' intentions to participate in activities at a soon-to-open senior center was explored using hierarchical multiple linear regression. The results indicated a direct influence of social network characteristics on older adults' intentions to participate in new activities, and some of the influence was mediated by functional features of social networks such as perceived support and companionship. Respondents who had smaller numbers of people in their networks and those with smaller proportions of network members who provided support had lower intentions to participate in new social activities, and thus should be targeted for intervention. It was also found that the construct of companionship and supportive relationship have independent associations with network characteristics and older adults' intentions to participate in activities. Because the older adults who had lower perceived availability of companionship had higher levels of intention to participate, these individuals are likely to be recruited successfully and benefit by promoting companionship among them. Additional research is needed to investigate other possible mechanisms linking social networks and intentions to participate, such as social influence from network members.

It makes recommendations for a collection of new, cross-national data on aging populationsâ€"data that will allow nations to develop policies and programs for addressing the major shifts in population age structure now occurring. These efforts, if made internationally, would advance our understanding of the aging process around the world.
