Individuals with these disorders often have poor social and vocational functional outcomes and reduced quality of life, even during times of symptom remission. Social cognition comprises a number of processes including emotion recognition, empathy and theory of mind (ToM), and recent research has indicated that these skills may be reduced in schizophrenia and bipolar disorder, and contribute to their reduced function. Despite this, the exact nature of these social cognitive deficits, their impact on quality of life, and possible underlying neural substrates, have not yet been established in these disorders. One brain network implicated in social cognition is the mirror system, however, research into this system in schizophrenia or bipolar disorder has been very limited to date. The goal of this thesis was to better understand the nature and impact of social cognitive deficits in schizophrenia and euthymic bipolar disorder, and comprehensively assess mirror system function as a possible contributor to these deficits. In this thesis, I first assessed a number of aspects of social cognition (emotion recognition, empathy and ToM), as well as cognitive, clinical, and quality of life variables in a schizophrenia, euthymic bipolar disorder, and healthy control group. The three groups were then compared using two putative measures of mirror system function (EEG and TMS). I found a significant reduction in cognitive ToM in both disorders, with the addition of a deficit in the recognition of anger in the schizophrenia sample, indicating some likely shared underlying brain pathology but also disorder-specific differences. Quality of life was not related to these social cognitive deficits in this sample, possibly due to other mediating factors. There was some evidence for underactive mirror systems in euthymic bipolar disorder but not schizophrenia, however, mirror system function was not strongly related to any of the social cognitive measures utilised in this study. The findings of this thesis make a valuable contribution to the understanding of social cognition in schizophrenia and bipolar disorder, and challenge the proposed importance of mirror systems for social cognition. Our findings indicate that social cognitive deficits should be considered in assessment and treatment planning in schizophrenia and bipolar disorder. In addition, this thesis highlights the need for further research aimed at understanding the neurobiology and true impact of social cognitive deficits. It is hoped that this knowledge will lead to better treatments targeting these deficits at both neurophysiological and behavioural levels, and ultimately improve patient outcomes and wellbeing.

Book excerpt: Background and Aims:Patients with bipolar disorders exhibit deficits in social and neurocognition even during remission and interfere with everyday life. This study aims to find association of executive functions and emotion and social cognition with clinical symptomatology ( mood, activity and speech) and global functioning. Methods:Thirty-five male and sixteen female patients who were diagnosed as BD type I , manic episode according to DSM-5 in the mental health service were included. All participants received a comprehensive neuropsychological assessment and ratings of depressive Montgomery-Asberg Depression Rating Scale (MADRS) and manic [Young Mania Rating Scale (YMRS)] symptoms at baseline and follow-up. Symptoms were examined in relation to neurocognitive performance.Results:In the first week of bipolar manic episode, YMRS scores of bipolar mania patients, elevated mood (item 1), increased motor activity-energy (item 2), speech (rate and amount) (item 6) and total scores were all significantly correlated with SSRT, spearmanu2019s rho value (r) as were 0,583, 0,504, 0,338 and 0,505 respectively. Correlation of item 1 with deliberation time (r=0,3) and rate of angry emotion recognition were found (r= -0,34). Item 2 and total score were positively correlated with total rate of bet (r=0,38 and r=0,35). General assesment of functioning score was correlated with deliberation time (r=-0,371) and total rate of bet (r=-0,443). Conclusions:Bipolar disorder mania clinical symptomatology was associated with neurocognition and social cognition. Cognitive and emotion recognition dysfunction relation to other key symptom dimensions and to diagnostic categories and general functioning will help clarify shared neural correlates and guide efforts toward personalized treatment approaches.

Book excerpt: Research has supported the impairment of both impulsivity and social cognition within bipolar disorder (BD). Despite this, additional research is necessary to clarify the manifestation of the sub-domains of these multi-faceted constructs. This study sought to examine the manifestation of the facets of impulsivity (i.e. trait behavioural) and social cognition (i.e. theory of mind social perception). Performances on validated measures of impulsivity and social cognition from a sample of 26 BD participants and 25 community controls were compared to observe for patterns indicative of impairment. The results of this examination suggest that euthymic and subthreshold BD individuals perform equal to controls regarding behavioural measures of impulsivity, theory of mind, and social perception. At the same time, levels of trait-impulsivity are statistically elevated compared to controls. These results suggest additional variables such as medication, symptom profile, and baseline functioning may mediate/moderate the manifestation of impulsivity and social cognitive impairments.

Book excerpt: Major Depressive Disorder (MDD) is a clinically and neurobiologically highly heterogenous mood disorder that consists of a variety of symptom clusters including mood symptoms, physical symptoms, and cognitive symptoms. While the traditional model of depression implicates mood as the primary symptom cluster, a more recently published conceptual understanding of depression has extended to also include cognitive symptoms. Moreover, the understanding of the central role of cognitive processes in emotion processing, social interaction, and pure cognitive performance has led to a model suggestion that places cognitive processing central to the pathophysiology of depression. Cognitive dimensions of depression have long been implicated in the nature of depression that are characterised typically by impaired cognitive and emotional processes. In the following volume the dimensions of cognitive function, emotion processing, and social cognitive processing are highlighted to comprehensively describe large parts of the clinical symptoms as well as the pathophysiology and biology of the brain-based disorder of depression.
