Book excerpt: With the development of effective antiretroviral therapies (ART) in the mid-1990s, HIV became a treatable although serious condition, and people who are adherent to HIV medications can attain normal or near-normal life expectancies. Because of the success of ART, people 50 and older now make up a majority of people with HIV in high-income countries and other places where ART is accessible. The aging of the HIV epidemic is a global trend that is also being observed in low- and middle-income countries, including countries in sub-Saharan Africa, where the greatest number of older people with HIV reside (3.7 million). While globally over half of older adults with HIV are in sub-Saharan Africa, we have little information about the circumstances, needs, and resiliencies of this population, which limits our ability to craft effective policy and programmatic responses to aging with HIV in this region. At present, our understanding of HIV and aging is dominated by information from the U.S. and Western Europe, where the epidemiology of HIV and the infrastructure to provide social care are markedly different than in sub-Saharan Africa. Aging with HIV in Sub-Saharan Africa addresses this gap in our knowledge by providing current research and perspectives on a range of health and psychosocial topics concerning these older adults from across this region. This volume provides a unique and timely overview of growing older with HIV in a sub-Saharan African context, covering such topics as epidemiology, health and functioning, and social support, as well as policy and program implications to support those growing older with HIV. There are very few published volumes that address HIV and aging, and this is the first book to consider HIV and aging in sub-Saharan Africa. Most publications in this area focus on HIV and aging in Uganda and South Africa. This volume broadens the scope with contributions from authors working in West Africa, Botswana, and Kenya. The range of topics covered here will be useful to professionals in a range of disciplines including psychology, epidemiology, gerontology, sociology, health care, public health, and social work.

Book excerpt: In sub-Saharan Africa, older people make up a relatively small fraction of the total population and are supported primarily by family and other kinship networks. They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.

Finally, it discusses the impact of HIV/AIDS in the education sector on the continent, with a particular focus on South Africa. In the first part of this paper I have chosen to categorise the discourses in circulation in relation to HIV/AIDS into three. Although political, historical and other discourses abound, I shall limit myself to three others to be able to discuss them fully and develop convincing arguments. The three categories shall be: medical or scientific discourse, socio-economic discourse and traditional discourse. The terms “medical” or “scientific” would therefore be used interchangeably, as the case may be, to mean the same discourse. Under the medical discourse I shall elucidate on the name, nature, mode of transmission and prevention. The socio-economic discourse shall revolve around poverty and social pressure. The final category, which is the traditional discourse, shall embody discourses surrounding patriarchal authority and certain mythological ideas embedded in most African cultures, which underpin the spread of HIV/AIDS in South Africa. The second part, which is a further development of this paper, I shall demonstrate how lack of sufficient attention regarding the above discourses, as discussed in the first part, has contributed to worsening the already bad situation of the HIV/AIDS pandemic in South Africa, not mentioning its neighbour countries of Sub-Saharan Africa. In this second part of the paper I shall explore some other reasons encouraging the increase in the spread of HIV/AIDS in South Africa. These reasons shall include the issue of gender roles favouring hegemonic masculinity, rape and certain myths and misconceptions that appear difficult to eradicate from the cultural fabric of the South African society.

The present book is based on research in a village in Tanzania about the role of social capital in mitigating AIDS impacts, at the level of the household and within the local community. It contributes to the current knowledge base on social capital by questioning general assumptions on the role of social capital in rural livelihoods in a context of high HIV/AIDS prevalence. Using a combination of qualitative and quantitative methods, the research yielded empirical evidence about the limitations of social capital as a resource for the poor. Both the generation and sustenance of social capital at household level are severely challenged by declining access to other livelihood assets as a consequence of HIV/AIDS. In a situation where HIV/AIDS is still shrouded in secrecy and stigma at the community level, a decline of trust in social relations and community institutions as well as a proliferation of witchcraft accusations, could be observed. This insightful publication provides a timely contribution to the discourse on the significance of social capital for the poor, ensuring that social scientists will never look at social capital in quite the same way.
