Book excerpt: Excerpt from Royal Society, Vol. 5: Reports of the Sleeping Sickness Commission We also continued the observations upon several Sleeping Sickness patients, and have reported a typical case occurring in a Persian which, at the time of writing the Report, we thought to be the first instance on record of the disease attacking an individual who was not a native of Africa. About the same time, although we heard of it only long afterwards, Sir P. Manson's patient (a European missionary), who had had trypanosomata in her blood for many months, died in England of Sleeping Sickness. Amongst the patients we had under observation was a boy who suffered from recurrent attacks of fever, rather like Relapsing Fever. He was sent up as a case of Sleeping Sickness, but we could get no cerebro-spinal fluid on lumbar puncture. On examining his blood, spirochaetes were found on at least two occasions, and as this was the first undoubted case of Spirochaete Fever occurring in that part of the world, we thought it worth putting on record. Since our case which was under observation in August, 1903, Drs. Hodges and Ross, of the Uganda Medical Service, have had other cases in native Waganda and in Indians domiciled in Uganda, which have been reported in the medical journals. Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease – attacking both motor and mental functions – appeared just as the separation of neurology and psychiatry had reached a critical point. Encephalitis lethargica sufferers presented an unprecedented combination of neurologic and psychiatric symptoms – including previously puzzling phenomena primarily associated with schizophrenia and hysteria, as well as behavioral changes and attention deficit disorders in children – that not only underscored the unity of mind and movement in the CNS, but also illuminated the critical role played by subcortical structures in consciousness and other higher mental functions that had formerly been associated with the soul and more recently presumed to be localized to the human cerebral cortex. Encephalitis lethargica exerted a greater influence on clinical and theoretic neuroscientific thought between the two World Wars than any other single disorder and had an enduring impact upon neurology and psychiatry.
