Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease – attacking both motor and mental functions – appeared just as the separation of neurology and psychiatry had reached a critical point. Encephalitis lethargica sufferers presented an unprecedented combination of neurologic and psychiatric symptoms – including previously puzzling phenomena primarily associated with schizophrenia and hysteria, as well as behavioral changes and attention deficit disorders in children – that not only underscored the unity of mind and movement in the CNS, but also illuminated the critical role played by subcortical structures in consciousness and other higher mental functions that had formerly been associated with the soul and more recently presumed to be localized to the human cerebral cortex. Encephalitis lethargica exerted a greater influence on clinical and theoretic neuroscientific thought between the two World Wars than any other single disorder and had an enduring impact upon neurology and psychiatry. As colonialism shifted from conquest to occupation, colonial scientists exercised much influence during periods of administrative uncertainty about the role and future of colonial rule. Impartial and objective science helped to justify the British civilizing mission in East Africa by muting the moral ambiguities and violence of colonial occupation. Africans' actions shaped systems of western scientific knowledge as they evolved in colonial contexts. Bridging what might otherwise be viewed as the disparate colonial functions of environmental and health control, sleeping sickness policy by the British was not a straightforward exercise of colonial power. The implementation of sleeping sickness control compelled both Africans and British to negotiate. African elite, farmers, and fishers, and British administrators, field officers, and African employees, all adjusted their actions according to on-going processes of resistance, cooperation and compromise. Interactions between colonial officials, their African agents, and other African groups informed African and British understandings about sleeping sickness, sleeping sickness control and African environments, and transformed Western ideas in practice.

Book excerpt: Human African trypanosomiasis (HAT), or sleeping sickness, is a parasitic infection that is almost invariably fatal unless treated. It is a neglected tropical disease that occurs in sub-Saharan Africa. The incidence of the disease is declining in response to intensive surveillance and control in endemic areas. As a result, HAT is among the neglected tropical diseases targeted by the World Health Organization (WHO) for elimination. WHO maintains exhaustive records of all declared cases; in 2018, a historically low number of cases (less than 1000) was reported. The remarkable progress in the control of gambiense HAT has relied on case-finding and curative treatment, a strategy that interrupts transmission by depleting the reservoir of parasites in humans. This has been combined occasionally with vector control activities. The subject of these guidelines, therefore, is of utmost importance for the continuation of progress to eliminate HAT. The recent approval of a new medicine (fexinidazole) for the treatment of gambiense HAT has opened new possibilities for the management of cases and thus warrants the new WHO recommendations contained herein. While studies of fexinidazole and other therapies are ongoing, these guidelines are considered interim guidelines until new information becomes available. This document focuses on the management of patients affected by gambiense HAT and constitutes an update to the WHO therapeutic guidance issued in 2013.
