Otro de los elementos esenciales para elfuncionamiento cohesionado y con garantías de calidad del Sistema Nacional deSalud, es la existencia de un verdadero sistema de información sanitaria que garantice la disponibilidad de la información y la comunicación recí-procas entre la Administración sanitaria del Estado y la de las comunidades autónomas, ysirva de punto de partida para la confección de protocolos clínicos y de Guías Integradas Asistenciales, y que posibilite la realización de un efectivo bench-marking. En este aspecto el libro aporta y describe también ampliamente envarios capítulos la importancia de los indicado-res tanto de actividad como decalidad asistencial con un enfoque final hacia la gestión clínica y el gobierno clínico, aspectos estos ampliamente descritos a lo largo de varios capítulos. En conclusión, creo que se trata de una obra de gran interés en el campo del mundo de la gestión sanitaria, esencial y de obligada lectura en aras de unfuncionamiento cohesionado y con garantías de calidad, y muy útil como guión yreferencia para poder introducir aquellas reformas necesarias que nos permitan reforzar la sostenibilidad, mejorar la eficiencia en la gestión, promover elahorro y las economías de escala, e introducir nuevas herramientas a través de las nuevas tecnologías con criterios de racionalización y eficiencia en la gestión, que permitan además una mayor homogeneización y estandarización del proceso, tanto en la gestión de estructuras sanitarias como en la clínica diaria.INDICE RESUMIDO: Sistemas sanitarios y reformas sanitarias. Unfortunately, there is evidence that current public health systems and services are not able to cope well with the modern challenges - new technologies, globalization, migration, bioterrorism, etc. In view of this, the WHO Western Pacific Region initiated studies on essential public health functions (EPHFs) in Fiji, Malaysia, and Vietnam. By drawing on research in other parts of the world, a framework and nine EPHFs were derived for these case studies. These nine EPHFs define more clearly and systematically the core areas of public health work for which governments are ultimately responsible. These EPHFs are also considered appropriate for further consultation and use in the Western Pacific Region. This publication is aimed at providing EPHF concepts and ideas that will help countries ensure that they develop and sustain a comprehensive approach to public health. The case studies may also provide ideas for other countries on methods that could be used to examine the current functioning of EPHFs and on different approaches for strengthening these essential functions in different health systems.

A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. There is a pressing need to submit HTA to in-depth critical scrutiny.
