Book excerpt: Introduction : la chirurgie d'élévation du sinus maxillaire avec comblement osseux (ou sinus lift) est une procédure courante visant à corriger le défaut osseux maxillaire pour permettre la pose d'implants dentaires. La greffe par os autogène est actuellement considérée comme le gold standard. Cependant sa quantité est limitée et nécessite un site donneur, ce qui augmente la morbidité. Le [beta]-TCP est un biomatériau synthétique couramment utilisé en chirurgie maxillo-faciale. Il semble induire une faible réponse inflammatoire. Le but de cette étude était d'étudier le degré d'inflammation de la muqueuse de Schneider après une chirurgie de sinus lift en fonction du type de biomatériau greffé. Matériel et Méthodes : 40 sinus (20 patients) ont été inclus et répartis en 2 groupes selon le biomatériau utilisé: biomatériau synthétique (groupe BTCP) ou os naturel (groupe BONE). Un groupe contrôle (groupe CTRL) a été constitué avec les sinus maxillaires non greffés. Douze mesures par sinus ont été réalisées sur les coupes scannographiques pré et post-opératoires permettant d'obtenir une épaisseur moyenne de la muqueuse de Schneider par sinus avant et après la chirurgie. Résultats : la muqueuse de Schneider est significativement plus épaisse dans les groupes BTCP et BONE après la chirurgie. Aucune différence n'a été retrouvée entre les groupes BTCP et BONE sur les mesures post-opératoires. Conclusion : nous avons donc constaté qu'un biomatériau synthétique ([beta]-TCP) n'induit pas plus d'inflammation que l'os naturel. Il constitue donc une alternative intéressante pour les chirurgies de sinus lift.
