Dans cette évolution, on peut par exemple noter l'élévation et le comblement du sinus maxillaire, technique prometteuse quand à l'aménagement du volume osseux implantaire. Cette technique fera l'objet de cette thèse, tant il est vrai que cet obstacle anatomique qu'est le sinus maxillaire nous limite fréquemment dans notre thérapeutique. Après un bref rappel historique, nous présenterons les éléments physiologiques et anatomiques importants en rapport avec cette technique. Le deuxième chapitre sera lui consacré au bilan pré-opératoire du patient, permettant de poser éventuellement les contre-indications générales et locales. Nous attacherons bien sûr une attention particulière aux moyens d'investigations des sinus maxillaires. Les paramètres à prendre en compte pour l'établissement d'un plan de traitement (qualité osseuse, volume osseux résiduel, espace prothétique et largeur osseuse...), ainsi que les principales techniques chirurgicales par abord crestal ou latéral du sinus, la prémédication du patient à l'intervention feront l'objet du troisième chapitre. Le quatrième chapitre aura pour but, après avoir présenté les différents mécanismes d'ossification des greffes osseuses, d'analyser les matériaux de comblement utilisables dans cette technique. Finalement, nous conclurons cette thèse par l'analyse des différents paramètres participant au succès d'une telle technique (choix de l'implant, du matériau de greffe, de la technique chirurgicale, et délais préconisés quant au moment de l'implantation et de la mise en charge).
