Book excerpt: Utilizing Norwegian linked register and survey data, while exploiting a discontinuity in public sick pay legislation, I show that the public sick pay compensation level causally affects male performance pay workers' sick leave days. Both male and female performance pay workers experience longer sick leaves when provided private supplementary sick pay compared with those being eligible for public sick pay only. This differential impact of the replacement rate on workers' sick leave rates reveals heterogeneous behavioural changes following public sick pay cuts, and this heterogeneity will be reinforced by the provision of employer-provided sick pay to attractive worker groups.

We use matched employer-employee panel data based on the period 2005-2008 containing information on employees' earnings, on their employers, their sick leave and their health care expenditure and a new data source describing the precise compensation scheme for each CBA. We consider three different sets of compensation variables and three different outputs and find each time a positive relationship between compensation and absence, except for executives for whom it is not significant.

Most end with a swift return to work, but over 300,000 people a year fall out of work onto health-related state benefits. This Review aims to stop as many people as possible from needlessly moving away from work because of ill health, and to find ways of improving the coherence, effectiveness and cost of the system for managing sickness absence. There are potential major gains for employers, who spend £9 billion a year sick pay and associated costs, and the state, which spends £13 billion annually on health-related benefits. Currently the majority of people seeking a medical certificate are signed off as completely unfit. Until this is addressed, employers cannot make adjustments to help those people whose illness is compatible with a return to work. The central recommendation is that the Government should establish an Independent Assessment Service (IAS) which would provide an in-depth assessment of an individual's physical and/or mental function. It would also provide advice about how an individual could be supported to return to work. The service would replace GP certification. Other recommendations include: tax relief for expenditure by employers on medical treatment/vocational rehabilitation; abolishing the Percentage Threshold compensation scheme and the record-keeping obligations under statutory sick pay; introduction of a job-brokering service for long-term sick employees; end the Employment and Support Allowance assessment phase so as to improve and speed up the benefits system; improved processes in Jobcentre Plus.

Book excerpt: This paper provides an analytical framework and uses data from the US and Germany to test for the existence of contagious presenteeism and negative externalities in sickness insurance schemes. The first part exploits high-frequency Google Flu data and the staggered implementation of U.S. sick leave reforms to show in a reduced-from framework that population-level influenza-like disease rates decrease after employees gain access to paid sick leave. Next, a simple theoretical framework provides evidence on the underlying behavioral mechanisms. The model theoretically decomposes overall behavioral labor supply adjustments ('moral hazard') into contagious presenteeism and noncontagious absenteeism behavior and derives testable conditions. The last part illustrates how to implement the model exploiting German sick pay reforms and administrative industry-level data on certified sick leave by diagnoses. It finds that the labor supply elasticity for contagious diseases is significantly smaller than for noncontagious diseases. Under the identifying assumptions of the model, in addition to the evidence from the U.S., this finding provides indirect evidence for the existence of contagious presenteeism.

Book excerpt: This Social Protection Committee (SPC) background report on sick leave and sick pay/sickness benefit schemes in the European Union sheds light on the huge variations in the way Member States address absence from work in case of temporary professional incapacity due to sickness. It is important to clearly define and distinguish three key notions. Sick leave concerns the right to be absent from work during sickness and return to one's job when recovered. Sick pay is the continued, time limited, payment of (part of) the worker’s salary by the employer during a period of sickness. A sickness benefit is provided by the social protection system and is paid as a fixed rate of previous earnings, or a flat-rate amount. All European Union (EU) Member States provide sick leave and sickness benefits. In most of them, salaried and waged employees may also — by law, collective agreement or at the discretion of the employer — be entitled to sick pay, either for an initial shorter period of absence or for the entire duration of sick leave. Sick pay and benefits schemes vary widely regarding their eligibility conditions, duration and replacement rates. Eligibility may indeed depend on whether people are dependently employed, self-employed or unemployed. They also often vary according to people’s employment status (e.g. civil servants, white collar employees or blue collar workers). In some countries, civil servants and white collar employees may be entitled to longer periods of sick pay than blue collar workers and often to full pay during sick leave. By contrast, blue collar workers typically have to rely on less favourable sick pay conditions. Replacement rates of compulsory sick pay vary from 25% (SK) to 100% (e.g. BE, FI). The sickness benefit replacement rates range between 50% and 100% of the gross (or in some cases the net) salary. In the case of some flat-rate sickness benefits the replacement level of an average wage can be estimated at around 20% (MT, UK).
