Book excerpt: The focus of this project is to significantly enhance the cultural competence of the healthcare providers embarking on short term immersion clinical experiences. The theoretical framework of Campinha-Bacote's Cultural Competence Model-The Process of Culture Competence in the Delivery of Health Care Services will be used to guide the project. The inventory tool, Assessing the Process of Cultural Competence Revised (IAPCC-R), will be purchased and used to assess for any statistical differences in cultural competence prior to the short-term immersion experience and again after the mission trip. For the qualitative research, a summative qualitative evaluation of the trip experiences measured qualitatively in the form of written answers to questions developed by this writer. Questions were developed utilizing Campinha-Bacote's Cultural Competence Model encompassing five components: cultural awareness, cultural skill, cultural knowledge, cultural encounter, and cultural desire. Due to COVID-19 restrictions on international travel and the inability for healthcare workers to go on medical missions, it was necessary to change the method of gathering the data to a qualitative questionnaire aimed for healthcare workers who have already been on a medical mission to Central America. A presentation video providing culturally sensitive education will be created and added to the onboarding process of future health care providers embarking on medical mission trips through ER Abroad's website. This author hypothesizes that the cultural competence educational intervention will add a higher level of cultural competence in health disparities, disease incidence and prevalence prior to the healthcare provider immersing into the medical mission trip. Every year, hundreds of thousands of people travel from wealthier to poorer countries to participate in short-term volunteer programs focused on health services. Churches, universities, nonprofit service organizations, profit-making "voluntourism" companies, hospitals, and large corporations all sponsor brief missions. Hoping to Help is the first book to offer a comprehensive assessment of global health volunteering, based on research into how it currently operates, its benefits and drawbacks, and how it might be organized to contribute most effectively. Given the enormous human and economic investment in these activities, it is essential to know more about them and to understand the advantages and disadvantages for host communities. Most people assume that poor communities benefit from the goodwill and skills of the volunteers. Volunteer trips are widely advertised as a means to "give back" and "make a difference." In contrast, some claim that health volunteering is a new form of colonialism, designed to benefit the volunteers more than the host communities. Others focus on unethical practices and potential harm to the presumed "beneficiaries." Judith N. Lasker evaluates these opposing positions and relies on extensive research—interviews with host country staff members, sponsor organization leaders, and volunteers, a national survey of sponsors, and participant observation—to identify best and worst practices. She adds to the debate a focus on the benefits to the sponsoring organizations, benefits that can contribute to practices that are inconsistent with what host country staff identify as most likely to be useful for them and even with what may enhance the experience for volunteers. Hoping to Help illuminates the activities and goals of sponsoring organizations and compares dominant practices to the preferences of host country staff and to nine principles for most effective volunteer trips.

Available in PDF, EPUB and Kindle. Book excerpt: Short-term medical mission (STMM) trips have steadily grown since the 1980s, providing care to a greater number of communities each year. With the increase in medical aid abroad, some researchers have begun to question if short term medical missions are causing more harm than good within a community. The literature review analyzed a total of 29 research articles focused on the impact that STMMs have in low-income communities. Findings were divide into four categories; STMM Evaluation, Education, Provider Experiences vs Community Needs, and Host-Provider Relationships. Significant challenges for effective STMM work included inadequate cultural education of volunteers, ineffective partnering with local healthcare providers, the attainment of experiences instead of the provision of care, and a lack of a standard evaluation tool to assess STMM work. Unfortunately, most of the current research consisted of volunteer perceptions of care, with very few research articles examining host healthcare and patient perspectives on care. Providers agreed that there needs to be greater research focused on patient and host observations of STMM care, as well as greater time dedicated to education and fostering relationships abroad. Current research recommended greater accountability in evaluating and reporting the effects of care provided by STMMs before they are deemed useful.
