Book excerpt: Since the respiratory airways branch to all parts of the lungs and ready access is provided through the nose or mouth, exploration of these passages for direct visualization or tissue sampling has long been a clinical challenge. This would be particularly helpful in diagnosing those pulmonary diseases that involve the bronchial tree or affect the surrounding lung parenchyma which prove difficult to diagnose or define by indirect methods. The pioneering efforts of Chevalier Jackson in 1918, using bismuth insufflation for radiologic visualization of the bronchial tree, and of Sicard and Forestier, who introduced poppy seed oil (lipiodol) in 1922, rapidly established bronchography as a practical radiologic diagnostic procedure. The initial enthusiasm was soon tempered by recognition of practical problems, and over the years the popularity of bronchography has waxed and waned as techniques were refined and new equipment, instrumentation and contrast agents evaluated. At the same time, alternative methods of diagnosis were being developed, notably sputum cytology, percutaneous needle aspira tion and biopsy, and bronchial brushing. In recent years, a number of medical and technologic developments have revived interest in transbronchial techniques and have made such a diagnostic approach more attractive. Improvements in topical airway anesthesia effectiveness have simplified passage of a variety of catheters, brushes, biopsy devices, fiberoptic or other bronchoscopic instruments along the bronchial passageways. Methods of guiding the catheter or other trans bronchial instrument toward the target site in the lung have also been refined.

Anyone who has come to know DE KOCK as a brilliant clini cian, or who has worked with him in the lung function labo ratory - he has made important contributions in this field also would have expected him to produce a monograph of this calibre. Now we have such a book. It contains the selected results of many years' solid work. Due attention is, of course, also accorded the merits of the various bronchoscopic techniques which provide the essential background to satisfactory results. In writing this book DE KOCK has succeeded in projecting a vivid image of the bronchi and the functional lung into our mind's eye. This work undoubtedly opens new prospects in improved care of our patients.

Book excerpt: The purpose of this series of volumes is to present a comprehensive view of the complications that result from the use of acceptable diagnostic and thera peutic procedures. Individual volumes will deal with iatrogenic complications involving (1) the alimentary system, (2) the urinary system, (3) the respiratory and cardiac systems, (4) the skeletal system and (5) the pediatric patient. The term iatrogenic, derived from two Greek words, means physician-in duced. Originally, it applied only to psychiatric disorders generated in the patient by autosuggestion, based on misinterpretation of the doctor's attitude and comments. As clinically used, it now pertains to the inadvertent side effects and complications created in the course of diagnosis and treatment. The classic categories of disease have included: (1) congenital and developmen tal, (2) traumatic, (3) infectious and inflammatory, (4) metabolic, (5) neoplastic, and (6) degenerative. To these must be added, however, iatrogenic disorders a major, although generally unacknowledged, source of illness. While great advances in medical care in both diagnosis and therapy have been accomplished in the past few decades, many are at times associated with certain side-effects and risks which may result in distress equal to or greater than the basic condi tion. Iatrogenic complications, which may be referred to as "diseases of medical progress," have become a new dimension in the causation of human disease.
