This is motivated by the clinical and basic science research requi- ment of obtaining more detailed physiological and pathological information about the body for establishing localized genesis and progression of diseases. Current research is also motivated by the fact that medical imaging is increasingly moving from a primarily diagnostic modality towards a therapeutic and interventional aid, driven by recent advances in minimal-access and robotic-assisted surgery. It was our great pleasure to welcome the attendees to MIAR 2004, the 2nd Int- national Workshop on Medical Imaging and Augmented Reality, held at the Xia- shan (Fragrant Hills) Hotel, Beijing, during August 19–20, 2004. The goal of MIAR 2004 was to bring together researchers in computer vision, graphics, robotics, and medical imaging to present the state-of-the-art developments in this ever-growing research area. The meeting consisted of a single track of oral/poster presentations, with each session led by an invited lecture from our distinguished international f- ulty members. For MIAR 2004, we received 93 full submissions, which were sub- quently reviewed by up to 5 reviewers, resulting in the acceptance of the 41 full - pers included in this volume.

Why do researchers exclude patients with primary progressive multiple sclerosis from enrolling in clinical trials? Please let me know if you hear of studies that I might be allowed to enter or treatments that I could try for my condition. " Thus, in recent years, the sad lament of the patient with primary progressive MS (PPMS). This variant, often in the guise of a chronic progressive myelopathy or, less commonly, progressive cerebellar or bulbar dysfunction, usually responds poorly to corticosteroids and rarely seems to benefit to a significant degree from intensive immunosuppressive treatments. In recent years, most randomized clin ical trials have excluded PPMS patients on two counts. Clinical worsening devel ops slowly in PPMS and may not be recognized during the course of a 2-or 3-year trial even in untreated control patients. This factor alone adds to the potential for a type 2 error or, at the very least, inflates the sample size and duration of the trial. In addition, there is mounting evidence that progressive axonal degeneration and neuronal loss (rather than active, recurrent inflammation) may be important components of the pathology in this form of the disease. Although contemporary trials are evaluating whether PPMS patients may benefit from treatment with the ~-interferons and glatiramer acetate, preliminary, uncontrolled clinical experi ence suggests that the results may not be dramatic.

Segmentation through registration and atlas label transfer has proven to be a flexible and fruitful approach as efficient, non-rigid image registration methods have become prevalent. Label transfer segmentation using multiple atlases has helped to bring statistical fusion, shape modeling, and meta-analysis techniques to the forefront of segmentation research. Numerous creative approaches have proposed to use atlas information to apply labels to brain anatomy. However, it is difficult to evaluate the relative advantages and limitations of these methods as they have been applied on very different datasets. This workshop provides a snapshot of the current progress in the field through extended discussions and provides researchers an opportunity to characterize their methods on standardized data in a grand challenge.
