Book Otitis Media with Effusion in Children

Download or read book Otitis Media with Effusion in Children written by David Chalmers and published by Cambridge University Press. This book was released on 1989 with total page 192 pages. Available in PDF, EPUB and Kindle. Book excerpt: Otitis media with effusion (OME) is one of the most common diseases of childhood, yet its long-term effects are only just beginning to be appreciated. Following a comprehensive review of the literature, David Chalmers and his co-authors detail a longitudinal, multidisciplinary study of over 1000 New Zealand children enrolled at three years and followed to age eleven. The changing prevalence of OME in the sample is described; evidence on a wide range of risk factors is examined; the problems of chronic perforation of the tympanic membrane, cholesteatoma, and ontological complications of surgical therapy are evaluated; and the developmental consequences of OME are traced over time. Early onset of bilateral OME is shown to have long-lasting consequences for language development, speech articulation, reading ability, and classroom behavior. The authors conclude from these results that there is an urgent need for the detection and management of early onset OME.

Book Screening Programmes for the Detection of Otitis Media with Effusion and Conductive Hearing Loss in Pre school and New Entrant School Children

Download or read book Screening Programmes for the Detection of Otitis Media with Effusion and Conductive Hearing Loss in Pre school and New Entrant School Children written by Lynley Cook and published by . This book was released on 1998 with total page 61 pages. Available in PDF, EPUB and Kindle. Book excerpt: Otitis media with effusion (OME) is common in preschool and new entrant school children, particularly in Maori and Pacific Island children. OME is frequently associated with conductive hearing loss. New Zealand has various health promotion strategies in place for OME including a national screening programme of 3 and 5-year-old children. There has been concern both internationally and within New Zealand over the effectiveness of these screening programmes. Using a systematic methodology, this review examined the evidence for screening programmes for OME and conductive hearing loss using Wilson and Jungner's screening criteria. Suitable screening tests are available for OME (tympanometry) and conductive hearing loss (audiometry). However, hearing testing of children under 4 years of age is more difficult. Screening programmes for OME and conductive hearing loss may vary greatly in the way they are constructed, and consequently vary in their sensitivity, specificity and predictive value. This makes it difficult to compare different screening programmes. Only a few studies have examined screening programmes for OME and conductive hearing loss and these were generally deficient in their design. They did not demonstrate screening programmes to be an effective strategy for OME and conductive hearing loss. Unfortunately, it is therefore not possible to conclude from the litterature reviewed in this report whether or nor screening programmes for OME and conductive hearing loss in preschool and school entrant children are an effective health strategy. It must be noted, that it cannot be concluded that current screening programmes for OME and conductive hearing loss in New Zealand are ineffective.

Book Surgical Management of Otitis Media with Effusion in Children

Download or read book Surgical Management of Otitis Media with Effusion in Children written by National Collaborating Centre for Women's and Children's Health (Grande Bretagne) and published by . This book was released on 2008 with total page 74 pages. Available in PDF, EPUB and Kindle. Book excerpt: Otitis media with effusion (OME) is a condition characterised by a collection of fluid within the middle ear without signs of acute inflammation. It is most common in young children, with a bimodal peak at 2 and 5 years of age. Eighty percent of children will have had a least one episode of OME by the age of 10 years. At age 7-8 years, about 8% of children will have middle ear effusions; this incidence increases in winter. The mean duration of effusions is 6-10 weeks but some cases are more persistent. OME is known to be a fluctuating condition with symptoms that vary with time and with age. The main symptom of OME is impaired hearing because the middle ear effusion causes a conductive hearing loss by reflection of the sound energy at the air-fluid interface. The diagnosis is based on suspicion of hearing loss, clinical history, clinical examination of the ears and appropriate audiometry and tympanometry. While most cases of OME will resolve spontaneously, some children will need intervention because of the effects of hearing loss. This intervention may take the form of educational and social action or the provision of a hearing aid to minimise the impact of the hearing loss. No non-surgical intervention has yet been shown conclusively to be of benefit. Surgical management usually takes the form of myringotomy and insertion of a ventilation tube (grommet), with or without adenoidectomy. Children with cleft palate are particularly susceptible to OME because of the impaired function of the Eustachian tube that results from the palatal anomaly, which in turn leads to a failure of middle ear ventilation. Similarly, children with Down's syndrome have a high incidence of OME, partly because of their impaired immunity and mucosal abnormality, with resulting susceptibility to ear infection. These groups of children need particular surveillance for OME so that proper action can be taken. This guideline attempts to bring together the evidence related to surgical management of OME in children younger than 12 years, and to advise on best and safest practice; treatment decisions need to balance potential benefits against potential risks, reflecting the fact that the degree of benefit usually depends upon the severity and persistence of the condition.

Book Measurement of Physiologic Health for Children Middle ear disease and hearing impairment

Download or read book Measurement of Physiologic Health for Children Middle ear disease and hearing impairment written by Kathleen N. Lohr and published by . This book was released on 1983 with total page 196 pages. Available in PDF, EPUB and Kindle. Book excerpt: Four upper respiratory conditions are investigated: middle ear disease (otitis media), hearing loss, tonsillitis, and colds. Data on prevalence and adverse consequences are used in the Health Insurance Experiment (HIE) to investigate the effect of different levels of health insurance on health and quality of care. This volume reviews pertinent medical literature, outlines the HIE definition of the diseases, describes HIE methods for measuring their presence, discusses enrollment results from the HIE population, and lists criteria for good medical care. Illustrative findings include: 11 percent of the children overall had had a middle ear infection within the previous year. Prevalence of current bilateral disease dropped with age, starting at 21 percent for ages 4-5 and decreasing to 9 percent at ages 12-13. Pain and distress and parental worry due to middle ear disease were the more frequently reported impacts. 1.2 percent of 1,464 children were reported as having a hearing deficit. Appendix A contains results for tonsillitis and colds.

Book Otitis media in children definition risk factors and pathogenesis

Download or read book Otitis media in children definition risk factors and pathogenesis written by Sics Editore and published by SICS Editore. This book was released on 2014-10-01 with total page 44 pages. Available in PDF, EPUB and Kindle. Book excerpt: About 40% of children experience one or more episodes of acute otitis media (AOM) by the age of one year and about 70% by the age of two years. It is normal that some middle ear effusion persists after AOM, even for several weeks. This must not be interpreted as a new episode of AOM.

Book Diagnosis of otitis media symptoms clinical examination and findings

Download or read book Diagnosis of otitis media symptoms clinical examination and findings written by Sics Editore and published by SICS Editore. This book was released on 2014-10-01 with total page 45 pages. Available in PDF, EPUB and Kindle. Book excerpt: Diagnosis is based on ear-related signs and symptoms. The child should not unnecessarily be labelled as a ”chronic ear patient”. The criteria for acute otitis media (AOM) are middle ear effusion bulging, cloudy, yellowish tympanic membrane with distorted or absent light reflex (pictures ) signs and symptoms of an acute infection. Redness alone is not a diagnostic finding (picture ). Diagnostic accuracy is greatly improved with the use of pneumatic otoscopy and tympanometry . It is normal that some middle ear effusion persists after AOM, even for several weeks . This must not be interpreted as a new episode of AOM. Wrong and uncertain diagnoses result in unnecessary courses of antibiotics and tympanostomy tube insertions.
