However, access to developmental screening is severely limited. This study evaluated an mHealth version of a standard developmental screening tool, Parents Evaluation Developmental Status (PEDS) and PEDS: Developmental Milestones (PEDS:DM) for use by community health workers (CHWs) in terms of (1) correspondence with conventional paper-based testing by a speech language pathologist (SLP) and (2) inter-rater reliability compared to an SLP. CHWs were trained in a primary health care (PHC) setting to administer the newly developed smartphone application version of the PEDS tools. A quantitative cross sectional, within subject research design was followed, by implementing a survey method. One SLP and two CHWs recruited 207 caregivers who were attending a baby wellness clinic. Caregivers were tested by one CHW using the smartphone application of the PEDS tools, a qualified SLP simultaneously recorded and scored the PEDS tools on the same participants. High positive (100%) and negative correspondence (96%) was found between the paper-based PEDS tools and the smartphone application PEDS tools and between the SLP and CHW. Almost perfect (Cohen s Kappa) inter-rater agreement between conditions was demonstrated (??=0.873 to ??=0.961). Outcomes of the smartphone application, operated by a CHW, corresponded closely to the gold standard PEDS tools operated by a health professional. Trained CHWs can conduct accurate developmental screening using the smartphone version of the PEDS tools. The smartphone version of the PEDS tools could be used as part of the Community Oriented Primary Care (COPC).

Book excerpt: In the past decade, American and Canadian pediatric societies have recommended that pediatric care clinicians follow a schedule of routine surveillance and screening for young children to detect conditions such as developmental delay, speech and language delays and disorders, and autism spectrum disorder. The goal of these recommendations is to ensure that children with these developmental issues receive appropriate referrals for evaluation and intervention. However, in 2015 and 2016, the US Preventive Services Task Force (USPSTF) and the Canadian Task Force on Preventive Health Care issued recommendations that did not support universal screening for these conditions. This occasional paper is designed to help make sense of the discrepancy between Task Force recommendations and those of the pediatric community in light of research and practice. To clarify the issues, this paper reviews the distinction between screening and surveillance; the benefits of screening and early identification; how the USPSTF makes its recommendations; and what the implications of not supporting screening are for research, clinical practice, and families.

Book excerpt: As many as half of all children with developmental delays are not identified before kindergarten, resulting in missed opportunities for intervention. Early intervention has been shown to improve long-term adult outcomes. One health department's care coordination program for children and pregnant women established a quality improvement project to increase early developmental screening. The problem statement was, "In health department care coordinators (HDCC), how does completing education compared to not completing education affect the amount of infant and child developmental screening performed within three months?" The primary aim was to increase the frequency of developmental screening, and the secondary aim was to improve HDCC self-rated confidence and knowledge in developmental screening and other related content. The plan was informed by a review of the literature, which revealed educational interventions proven successful in changing healthcare worker behavior. Strategies gleaned from the literature included the presumptive recommendation approach, group script development, role-playing, and ongoing post-education group discussions and feedback. The Precaution Adoption Process Model was incorporated as the theoretical framework equipping participants to avoid and/or manage potential resistance from parents about developmental screening. The health department's leadership was highly engaged in planning and implementation. All seven HDCC on staff participated in two education sessions. The first session focused on didactic content and the second on screening administration and scoring with the Ages and Stages Questionnaire. Each participant completed a pre-and post-survey of self-rated confidence and knowledge. The primary and secondary aims were achieved with statistically significant improvements. Developmental screening performance increased to 25% of eligible families from 11% at the baseline. The total score on pre-and post- confidence and knowledge increased from 2.4 to 3.6 on a four-point scale. The project demonstrated the value of education-focused quality improvement initiatives using a multifaceted approach informed by the literature. Positive enablers for ongoing improvement and sustainment that were not gleaned from the literature included a group activity to calculate the number of children with delays likely to be missed on HDCC caseloads and individual performance data given to HDCC in the post-education period.

Book excerpt: Objective: Developmental screening of infants and young children increases early identification of developmental delay and disability and creates an opportunity to connect families with needed services. Ensuring screening for all children requires collaboration across the medical, educational, childcare and family resource sectors. This study examines the approach and activities of a collaborative community work group in Yakima County, WA, to create a system to improve access to developmental screening and related resources to all children in the county. Methods: An array of child health and early learning professionals gathered in January 2010 to discuss developmental screening in the county. From this large group, a smaller work group, the Yakima County Developmental Screening Work Group, was formed in January 2011 and met regularly to formulate a county-wide plan for a universal developmental screening system. This case study examined the work of the Work Group through key informant interviews, review of meeting minutes and grant-related documents, and direct observation. The degree to which the group collaborated across sectors was evaluated using the framework of Collective Impact, a model for cross-sector collaboration. Results: Within-group collaboration was marked by Work Group members' strong personal commitment to the goal of developmental screening, their ability to think at a systems level, and their belief in the "whole child" approach to child wellness. Collaboration between the Work Group and partner agencies involved four strategies: 1) visiting the partner's site, 2) customizing presentations about child development, 3) listening to concerns, and 4) supporting partners' knowledge of their service population and their decisions regarding screening implementation activities. The Collective Impact framework indicated the Work Group's strengths in the areas of continuous communication and mutually-reinforcing activities. Backbone support and a shared measurement system were the most challenging components of the Collective Impact framework to fulfill. Conclusions: Systems level thinking and a flexible approach towards supporting community partners as they implement screening in their settings were key strategies of the Yakima County Developmental Screening Work Group. Efforts to develop systems of universal developmental screening in other communities should prioritize funding for backbone support staff and shared measurement to ensure effective coordination and accountability among collaborating agencies.
