The story of what has become popularly known as the Fetterman Fight, near Fort Phil Kearney in present-day Wyoming in 1866, is based entirely on this infamous declaration attributed to Capt. William J. Fetterman. Historical accounts cite this statement in support of the premise that bravado, vainglory, and contempt for the fort's commander, Col. Henry B. Carrington, compelled Fetterman to disobey direct orders from Carrington and lead his men into a perfectly executed ambush by an alliance of Plains Indians. In the aftermath of the incident, Carrington's superiors--including generals Ulysses S. Grant and William T. Sherman--positioned Carrington as solely accountable for the "massacre" by suppressing exonerating evidence. In the face of this betrayal, Carrington's first and second wives came to their husband's defense by publishing books presenting his version of the deadly encounter. Although several of Fetterman's soldiers and fellow officers disagreed with the women's accounts, their chivalrous deference to women's moral authority during this age of Victorian sensibilities enabled Carrington's wives to present their story without challenge. Influenced by these early works, historians focused on Fetterman's arrogance and ineptitude as the sole cause of the tragedy. In Give Me Eighty Men, Shannon D. After LSD arrived in the United States in 1949, the drug's therapeutic promise quickly captured the interests of psychiatrists. In the decade that followed, modern psychopharmacology was born and research into the drug's perceptual and psychological effects boomed. By the early 1960s, psychiatrists focused on a particularly promising treatment known as psychedelic therapy: a single, carefully guided, high-dose LSD session coupled with brief but intensive psychotherapy. Researchers reported an astounding 50 percent success rate in treating chronic alcoholism, as well as substantial improvement in patients suffering from a range of other disorders. Yet despite this success, LSD officially remained an experimental drug only. Research into its effects, psychological and otherwise, dwindled before coming to a close in the 1970s. In The Trials of Psychedelic Therapy, Matthew Oram traces the early promise and eventual demise of LSD psychotherapy in the United States. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.
