Book excerpt: La loi du 9 août 2004 relative à la politique de santé publique autorise les sages-femmes à pratiquer la consultation postnatale "si la grossesse a été normale et l'accouchement a été eutocique". L'objectif de ce mémoire était donc d'établir un état des lieux sur la réalisation de cet examen par les sages-femmes libérales de Lorraine. Pour se faire deux études ont été réalisées: l'une auprès des sages-femmes afin d'évaluer leur pratique et leurs difficultés, l'autre auprès d'accouchées pour connaître leur niveau d'information et leurs attentes concernant cet examen. Nous avons ainsi pu constater que la consultation postnatale s'inscrit difficilement dans l'activité des sages-femmes en raison d'une pratique insuffisante de suivi prénatal et d'un manque d'information des patientes. Dans l'avenir, la pénurie de gynécologues-obstétriciens et la critique de l'hyper-médicalisation par les patientes devrait contribuer à une plus grande pratique de la consultation postnatale par les sages-femmes. Ceci permettrait par conséquent d'obtenir une plus grande reconnaissance de la profession de sage-femme autant dans le milieu médical qu'au sein de la société. Des pré-requis semblent cependant indispensables: une meilleure information des patientes sur nos compétences médicales et une plus grande motivation des sages-femmes elles-mêmes concernant cet examen.

Book excerpt: En France, 3,3 millions d'hommes et de femmes sont touchés par desproblèmes d'infertilité, soit 1 couple sur 4. Parallèlement à l'augmentation de ces difficultés de conception, le nombre de couples ayant recours à l'Assistance Médicales à la Procréation (AMP) s'accroît également. Les sages-femmes sont concernées par la nouvelle stratégie nationale de lutte contre l'infertilité, en lien avec le rapport sur l'infertilité de 2022. Le but de notre étude était de déterminer la place et le rôle de la sage-femme avant l'orientation des couples infertiles vers les centres d'AMP ou de professionnels de santé spécialisés. Pour cela, nous avons réalisé une étude descriptive des pratiques actuelles des sages-femmes libérales et territoriales de Seine-Maritime concernant la prise en charge des couples infertiles. Nous avons pu comparer notre état des lieux à la revue de la littérature en utilisant les 69 réponses de notre questionnaire. En pratique, les sages-femmes sont confrontées à prendre en charge des couples hypofertiles. Elles peuvent prescrire des bilans pour explorer une infertilité et ont un rôle important de prévention sur les facteurs nuisibles à la fertilité. Cependant, elles éprouvent des difficultés dans le dépistage et se sentent moyennement à l'aise pour mener ce genre de consultation. Pour répondre à leurs problèmes, nous avons élaboré une fiche pratique pour mener des consultations d'infertilité et leur proposons une courte formation au CHU de Rouen. Une modification du programme pédagogique des études de sages-femmes serait également à envisager.

Tout est question de contexte : médical, social et psychologique. "Il appartient donc au médecin obstétricien référent, ou à l'équipe obstétricale référente, qui connaît bien la patiente, d'évaluer, au cas par cas, en fonction de la situation et de son expérience personnelle, la possibilité du suivi à domicile par la sage-femme libérale". Ce type de suivi suppose des compétences professionnelles et engage des responsabilités, morale d'abord, ainsi que celles relevant d'une observation sans faille de la loi, du code de déontologie et des règlements, comme ceux de la Sécurité Sociale que l'on pourra trouver dans cette étude en annexes. Comment se déroule le suivi à domicile ? Des objectifs à la naissance de l'enfant, en passant par l'organisation de la première visite et des suivantes dans leurs différents volets, sans oublier la circulation de l'information ascendante, remontant au médecin obstétricien, ni celle descendante, que la sage-femme donne à la patiente dans un but double d'information et de prévention, les maîtres mots sont, entre autres : sécurité, sécurisation, prévention, information, travail en équipe et, au delà du contrat, économie publique. Les enquêtes menées auprès des patientes, des sages-femmes ainsi que des médecins montrent clairement la satisfaction commune, pour des motifs différents, à l'endroit du suivi à domicile des grossesses à risques. L'aspect de la courbe illustrant la population des sages-femmes libérales en France pourrait inquiéter. En effet, durant les dix années 1977-1987, les effectifs sont passés de 2500 à 1250 (chiffres arrondis). La dernière décennie voit une stabilisation se dessiner autour de 1250. Y a-til désaffection par manque d'information, de courage ç s'engager de la part de jeunes diplômés ? L'effectif des sages-femmes libérales est appelé, dans la conjoncture des années de fin de siècle, à une certaine renaissance dont l'auteur ne doute pas puisque 1695 sages-femmes libérales sont en activité en France au 1er janvier 1996 - chiffre à rapprocher des 1250 de l'année 1987. La dernière partie de l'étude montre les intérêts, dans des domaines variés, du suivi à domicile. Le travail de l'équipe, dans laquelle les obstétriciens référents pratiquant en maternité et les sages-femmes libérales composent un microcosme oeuvrant dans le sens d'une sécurité accrue de la santé de la mère et de l'enfant, loin d'occasionner des frais supplémentaires à la société, par les hospitalisations coûteuses qu'elle permet d'éviter lorsqu'elles ne sont pas impératives, contribue à réaliser à moindres frais un service judicieusement organisé.
