Book excerpt: The Dept. of Energy (DoE) oversees contractors that operate more than 200 "high-hazard" nuclear facilities, where an accident could have serious consequences for workers and the public. DoE is charged with regulating the safety of these facilities. A key part of DoE's self-regulation is the Office of Health, Safety and Security (HSS), which develops, oversees, and helps enforce nuclear safety policies. This is the only DoE safety office intended to be independent of the program offices, which carry out mission responsibilities. This report examines: (1) the extent to which HSS meets elements of effective independent nuclear safety oversight; and (2) the factors contributing to any identified shortcomings with respect to these elements. Book excerpt: DOE-RL has set clear expectations for a cost-effective approach for achieving compliance with the Nuclear Safety Management requirements (10 CFR 830, Nuclear Safety Rule) which will ensure long-term benefit to Hanford. To facilitate implementation of these expectations, tools were developed to streamline and standardize safety analysis and safety document development resulting in a shorter and more predictable DOE approval cycle. A Hanford Safety Analysis and Risk Assessment Handbook (SARAH) was issued to standardized methodologies for development of safety analyses. A Microsoft Excel spreadsheet (RADIDOSE) was issued for the evaluation of radiological consequences for accident scenarios often postulated for Hanford. A standard Site Documented Safety Analysis (DSA) detailing the safety management programs was issued for use as a means of compliance with a majority of 3009 Standard chapters. An in-process review was developed between DOE and the Contractor to facilitate DOE approval and provide early course correction. As a result of setting expectations and providing safety analysis tools, the four Hanford Site waste management nuclear facilities were able to integrate into one Master Waste Management Documented Safety Analysis (WM-DSA).

Book excerpt: In June of this year, the Department of Energy (DOE) issued directives DOE O 414.1C and DOE G 414.1-4 to improve quality assurance programs, processes, and procedures among its safety contractors. Specifically, guidance entitled, ''Safety Software Guide for use with 10 CFR 830 Subpart A, Quality Assurance Requirements, and DOE O 414.1C, Quality Assurance, DOE G 414.1-4'', provides information and acceptable methods to comply with safety software quality assurance (SQA) requirements. The guidance provides a roadmap for meeting DOE O 414.1C, ''Quality Assurance'', and the quality assurance program (QAP) requirements of Title 10 Code of Federal Regulations (CFR) 830, Subpart A, Quality Assurance, for DOE nuclear facilities and software application activities. [1, 2] The order and guide are part of a comprehensive implementation plan that addresses issues and concerns documented in Defense Nuclear Facilities Safety Board (DNFSB) Recommendation 2002-1. [3] Safety SQA requirements for DOE as well as National Nuclear Security Administration contractors are necessary to implement effective quality assurance (QA) processes and achieve safe nuclear facility operations. DOE G 414.1-4 was developed to provide guidance on establishing and implementing effective QA processes tied specifically to nuclear facility safety software applications. The Guide includes software application practices covered by appropriate national and international consensus standards and various processes currently in use at DOE facilities. While the safety software guidance is considered to be of sufficient rigor and depth to ensure acceptable reliability of safety software at all DOE nuclear facilities, new nuclear facilities are well suited to take advantage of the guide to ensure compliant programs and processes are implemented. Attributes such as the facility life-cycle stage and the hazardous nature of each facility operations are considered, along with the category and level of importance of the software. The discussion provided herein illustrates benefits of applying the Safety Software Guide to work activities dependent on software applications and directed toward the design of new nuclear facilities. In particular, the Guide-based systematic approach with software enables design processes to effectively proceed and reduce the likelihood of rework activities. Several application examples are provided for the new facility.

Book excerpt: The U.S. Department of Energy Idaho Operations Office (DOE-ID) provided guidance per DOE-ID Orders 420.C, "Safety Basis Review and Approval Process," and 420.D, "Requirements and Guidance for Safety Analysis," for conducting safety analysis for facilities and activities that do not meet either the nuclear facility criteria or the criteria for not requiring additional safety analysis (NRASA). These facilities and activities are thus designated as "other than nuclear" (OTN), and hazard analyses are performed using a graded approach. This graded approach is done in accordance with DOE-ID Order 420.D. DOE-ID guidance is used to format these OTN facilities and activities into 3-chapter documents, rather than the 17-chapter format specified in DOE-STD-3009-94, "Preparation Guide for U.S. Department of Energy Nonreactor Nuclear Facility Safety Analysis Reports."

Book excerpt: Congress established DNFSB in 1988 to provide independent analysis and recommendations to the Department of Energy (DOE) to ensure adequate protection of public health and safety at defense nuclear facilities. DNFSB consists of a five-member Board, which currently has two vacancies, and about 103 technical, legal, and administrative staff. Until fiscal year 2012, DNFSB was not required to have an IG and did not have routine independent oversight by any other federal entity. GAO was asked to review the operations and oversight of DNFSB. This report examines the extent to which DNFSB had (1) policies and procedures governing the activities of the Board and technical staff; (2) assessed its internal controls; (3) meeting and voting practices that are transparent to the public; and (4) taken steps to obtain IG oversight and the results of those steps. GAO reviewed relevant federal laws, regulations, and guidance; analyzed DNFSB documents, including records of internal control assessments; interviewed officials from DNFSB, NRC-OIG, and DOE; and gathered information from 14 other federal agency IGs. GAO recommends, among other things, that DNFSB document its internal control assessment activities and implement a policy to publicly disclose the results of Board votes.
