Book excerpt: The nuclear power community in the United States is moving to modernize aging power plant control rooms as well as develop control rooms for new reactors. New generation control rooms, along with modernized control rooms, will rely more heavily on automation and computerized procedures. Of particular importance to the Nuclear Regulatory Commission (NRC) is the impact such modernizations or new technologies will have upon operator performance and reliability in these safety-critical control room environments. One specific area of interest is the effect that various complexities in the control room have on operator performance and reliability. This research identifies various definitions of complexity and characterizes complexity in the nuclear power plant (NPP) domain, focusing on the common complexity dimensions of number, variety, and interconnections. Based on this characterization of complexity, a comprehensive list of complexity sources within the NPP control room is presented, along with a novel approach to describe complexity source interconnections. Understanding the sources of complexity in advanced NPP control rooms and the associated effects on human reliability is critical for ensuring safe performance of both operators and the entire system. However, most of the previous approaches in investigating complexity typically focus on either objective or subjective views of complexity, and a systematic approach that considers both views is missing from previous approaches. This research provides a novel methodology to assess the sources of complexity in NPP control rooms both objectively and subjectively while understanding the difference between the two and introduces a systems theoretic descriptive model of these sources of complexity, leveraging network theory. Finally a method is introduced to investigate the differences between the complexity views of different groups of NPP stakeholders. Incident report databases and in particular, 22 nuclear incidents in the Human Event Repository and Analysis (HERA) database were parsed to find objective evidence for the identified sources. Using this evidence-based approach, some of the potential interactions between these sources could be captured. A network called "Complexity Source Network" (CSN) was created for each incident in HERA to present the sources (nodes) and potential interactions between them (links). An ensemble of networks was developed consisting of 22 CSNs, one for each of the incidents in HERA. A tool called CXViz was developed to visualize and analyze the CSNs. Using the aggregate network (22 CSNs combined) the most common sources and interactions were identified. The complexity views of three groups of stakeholders, namely Operators, Designers and NRC Reviewers, were collected using a survey tool called CXSurvey. Using this tool, the interviewees were asked to rate the identified sources of perceived complexity and to rank the top five in terms of contribution to perceived complexity. Data gathered from 16 operators, 8 designers and 3 NRC reviewers were collected and the top five sources identified by each group were compared to the top five most supported sources from the 22 incidents in HERA. The results show large variations between the subjective views of the operators and designers and the objective top five. In addition, the subjective source rating of the three groups of stakeholders were compared. The results show large variations between the complexity views between the stakeholders on some controversial sources such as boredom, and training.

Book excerpt: Life extension beyond 60 years for the U.S operating nuclear fleet requires that instrumentation and control (I & C) systems be upgraded to address aging and reliability concerns. It is impractical for the legacy systems based on 1970's vintage technology operate over this extended time period. Indeed, utilities have successfully engaged in such replacements when dictated by these operational concerns. However, the replacements have been approached in a like-for-like manner, meaning that they do not take advantage of the inherent capabilities of digital technology to improve business functions. And so, the improvement in I & C system performance has not translated to bottom-line performance improvement for the fleet. Therefore, wide-scale modernization of the legacy I & C systems could prove to be cost-prohibitive unless the technology is implemented in a manner to enable significant business innovation as a means of off-setting the cost of upgrades. A Future Vision of a transformed nuclear plant operating model based on an integrated digital environment has been developed as part of the Advanced Instrumentation, Information, and Control (II & C) research pathway, under the Light Water Reactor (LWR) Sustainability Program. This is a research and development program sponsored by the U.S. Department of Energy (DOE), performed in close collaboration with the nuclear utility industry, to provide the technical foundations for licensing and managing the long-term, safe and economical operation of current nuclear power plants. DOE's program focus is on longer-term and higher-risk/reward research that contributes to the national policy objectives of energy security and environmental security . The Advanced II & C research pathway is being conducted by the Idaho National Laboratory (INL). The Future Vision is based on a digital architecture that encompasses all aspects of plant operations and support, integrating plant systems, plant work processes, and plant workers in a seamless digital environment to enhance nuclear safety, increase productivity, and improve overall plant performance. The long-term goal is to transform the operating model of the nuclear power plants (NPP)s from one that is highly reliant on a large staff performing mostly manual activities to an operating model based on highly integrated technology with a smaller staff. This digital transformation is critical to addressing an array of issues facing the plants, including aging of legacy analog systems, potential shortage of technical workers, ever-increasing expectations for nuclear safety improvement, and relentless pressure to reduce cost. The Future Vision is based on research is being conducted in the following major areas of plant function: (1) Highly integrated control rooms; (2) Highly automated plant; (3) Integrated operations; (4) Human performance improvement for field workers; and (5) Outage safety and efficiency. Pilot projects will be conducted in each of these areas as the means for industry to collectively integrate these new technologies into nuclear plant work activities. The pilot projects introduce new digital technologies into the nuclear plant operating environment at host operating plants to demonstrate and validate them for production usage. In turn, the pilot project technologies serve as the stepping stones to the eventual seamless digital environment as described in the Future Vision.

The US Nuclear Regulatory Commission reviews the human engineering aspects of such control rooms to ensure that they are designed to good human factors engineering principles and that operator performance and reliability are appropriately supported in order to protect public health and safety. This paper describes the rationale and general approach to the development of a human factors review guideline for ACRs. The factors influencing the guideline development are discussed, including the review environment, the types of advanced technologies being addressed, the human factors issues associated with advanced technology, and the current state-of-the-art of human factors guidelines for advanced human-system interfaces (HSIs). The proposed approach to ACR review would track the design and implementation process through the application of review guidelines reflecting four review modules: planning, design process analysis, human factors engineering review, and dynamic performance evaluation. 21 refs.

Book excerpt: Control rooms for advanced nuclear power plants use advanced human-system interface (HSI) technologies that may have significant implications for plant safety in that they will affect the operator's overall role in the system and the ways in which operators interact with the system. The US Nuclear Regulatory Commission (NRC) reviews HSIs to ensure that they are designed to accepted human factors engineering (HFE) principles. The principal review guidance, however, is more than ten-years old (US NRC, 1981). Accordingly, an Advanced HSI Design Review Guideline (DRG) was developed to provide criteria for these reviews. The DRG contains seven major sections: Information Display, User-System Interaction, Process Control and Input Devices, Alarms, Analysis and Decision Aids, Inter-Personnel Communication, and Workplace Design (see O'Hara & Brown, 1993). The purpose of this paper is to describe the methodology for DRG use.
