Accumulating evidence supports the importance of cell adhesion molecule expression as an initiating process in tissue inflammation. Despite progress made to date, much is still unknown about the exact mechanisms responsible for this inflammatory response. Scientists have been working to understand the selective cell recruitment operating in allergic disease with the hope of discovering therapeutic intervention strategies that will prevent the accumulation of unwanted cells in inflamed airways. Research has been directed at developing various approaches to generate specific antagonists. Some approaches under study interrupt airway inflammation in its early stages during leukocyte-endothelial interactions. Other approaches inhibit cell recruitment at the endothelial wall. Many studies have been done, both in vivo and in vitro, and the advances that have been made suggest that these therapeutic interventions may be the keys to controlling and, possibly, curing asthma and allergic reactions.

Initially, PI3K inhibitors such as Wortmannin, LY294002 and Rapamycin were used to establish a central role for Pi3K pathway in immune cells. Considerable progress in understanding the role of this pathway in cells of the immune system has been made in recent years, starting with analysis of various PI3K and Pten knockout mice and subsequently mTOR and Foxo knockout mice. Together, these experiments have revealed how PI3Ks control B cell and T cell development, T helper cell differentiation, regulatory T cell development and function, B cell and T cell trafficking, immunoglobulin class switching and much, much more. The PI3Kd inhibitor idelalisib has recently been approved for the treatment of B cell lymphoma. Clinical trials of other PI3K inhibitors in autoimmune and inflammatory diseases are also in progress. This is an opportune time to consider a Research Topic considering when what we have learned about the PI3K signalling module in lymphocyte biology and how this is making an impact on clinical immunology and haematology.

It is suggested that many diseases are initiated and their progress is influenced by inflammatory molecules and a decrease in the production and/or action of anti-inflammatory molecules and this imbalance between pro- and anti-inflammatory molecules seems to have been initiated in the perinatal period. This implies that strategies to prevent and manage various adult diseases should start in the perinatal period. An alteration in the metaolism of essential fatty acids and their anti-inflammatory molecules such as lipoxins, resolvins, protecitns, maresins and nitrolipids seems to play a major role in the pathobiology of several adult diseases. Based on these concepts, novel therapeutic approaches in the management of insulin resistance, obesity, type 2 diabetes mellitus, metabolic syndrome, cancer, lupus, rheumatoid arthritis and other auto-immune diseases are presented. Based on all these evidences, a unified concept that several adult diseases are due to an alteration in the balance between pro- and anti-inflammatory molecules is discussed and novel methods of their management are presented.
