Cette étude montre comment s'organise la réponse des officines faces aux attentes de la collectivité en matière d'action sanitaire, notamment en Haute-Normandie. Une enquête a montré que dans la très grande majorité, les pharmaciens d'officine sont prêts à assurer les tâches de collectes, essentielles pour le service rendu aux usagers, qui concernent les médicaments et les déchets d'activités de soins à risque infectieux.

Book excerpt: Le médicament n'est pas un produit comme les autres, il n'est pas non plus un déchet comme les autres puisqu'il présente un danger sanitaire et environnemental. Dans le cadre de la valorisation des Déchets Issus des Médicaments (DIM), les pharmaciens d'officines ont un rôle majeur à jouer via des filières d'éliminations spécifiques désormais parfaitement identifiées. Ce travail propose d'analyser les connaissances actuelles des pharmaciens réunionnais face à l'élimination des déchets retrouvés à l'officine : Médicaments Non Utilisés (MNU), Déchets d'Activités de Soins à Risques Infectieux (DASRI) et Stupéfiants. Les pratiques quotidiennes sont évaluées via un questionnaire auto-administré contenant 10 cas pratiques. Les 247 pharmacies de l'Ile de la Réunion ont été sollicité, 44 pharmaciens ont répondu soit un taux de réponses de 7,12%. La filière des MNU gérée par CYCLAMED est la filière la mieux respectée avec la procédure d'élimination des stupéfiants. En effet, en moyenne 65,9% des déchets correspondant à la définition de MNU sont correctement éliminés par les pharmaciens et 88,6% pour les produits stupéfiants. Ce sont les assimilés stupéfiants et les Piquants Coupants Tranchants (PCT) qui sont fréquemment source de confusion. Les assimilés stupéfiants sont confondus avec les stupéfiants et les médicaments PCT se retrouvent dans la filière des MNU. Le conditionnement et le statut des produits n'est pas toujours bien identifié ce qui cause des erreurs au moment du tri. Ainsi, nous avons proposé deux affiches d'aide au tri des DIM : Une destinée aux pharmaciens et l'autre destinée aux patients.

Il doit donc informer les patients des modalités de retour et de tri.Dans ce travail, une étude réalisée sur 27 patients montre que les gestes de tri relatifs aux DASRI sont globalement bien compris. Cependant, lors de cette étude, il a été constaté que l'équipe officinale n'était pas toujours préparée aux questions des patients. Une brochure informative a donc été élaborée, à la fois pour être donnée aux patients afin de les sensibiliser à la démarche de tri, mais aussi pour servir d'appui et de formation à l'équipe officinale.Les déchets issus de médicaments sont principalement la conséquence d'une utilisation déraisonnée et d'une mauvaise gestion des traitements. Il est néanmoins possible de réduire la formation de ces déchets en agissant à plusieurs niveaux du cycle de vie du médicament : de sa conception par les industriels jusqu'à son élimination.

Book excerpt: Evaluates the carcinogenic risks to humans posed by the use of four antiretroviral agents four DNA topoisomerase II inhibitors used in the treatment of cancer and an additional three pharmaceutical agents (hydroxyures phenolphthalein and vitamin K substances). The volume marks the first IARC evaluation of nucleoside analogs that act as antiviral agents. The evaluation responds in part to recent findings that zidovudine (AZT) an effective antiretroviral agent now being given to pregnant HIV-infected women to prevent maternal-to-fetal transmission of the virus is a transplacental carcinogen in mice. The opening monograph evaluates the carcinogenicity to humans of the antiretroviral nucleoside analogs zidovudine (AZT) zalcitabine (ddC) and didanosine (ddI) and the antiherpesvirus drug aciclovir. Of these aciclovir and didanosine could not be classified on the basis of available data. For zidovudine transplacental administration to mice resulted in an increased incidence and multiplicity of lung and liver tumours and in an increased incidence of female reproductive tract tumours in one study but not in another involving treatment at a lower dose. Despite observation of toxic effects in some studies of humans human carcinogenicity data were judged to provide inadequate evidence of carcinogenicity in humans. Zidovudine was classified as possibly carcinogenic to humans. Similar weaknesses in human carcinogenicity data for zalcitabine which consistently induces thymic lymphomas in mice resulted in its classification as possibly carcinogenic to humans. The second monograph evaluates four DNA topoisomerase II inhibitors: etoposide teniposide mitoxantrone and amsacrine. Of these etoposide - one of the most widely used and effective cytotoxic drugs in combination therapy - was classified as probably carcinogenic to humans and etoposide in combination with cisplatin and bleomycin was judged to be carcinogenic to humans. Teniposide was classified as probably carcinogenic to humans and mitoxantrone and amsacrine were classified as possibly carcinogenic to humans. Of the three pharmaceutical agents evaluated in the final monograph hydroxyurea which is widely used in cancer treatment and increasingly in combination with didanosine in HIV infection could not be classified. Phenolphthalein a widely used laxative now being withdrawn from the market in many countries because of toxicological concerns was classified as possibly carcinogenic. Vitamin K substances could not be classified on the basis of available evidence.
