Available in PDF, EPUB and Kindle. Book excerpt: Kidney transplantation from a living donor provides the best chance for successful renal replacement therapy. However patient’s safety remains of paramount importance and complications are unacceptable. Laparoscopic donor nephrectomy (LDN) has been proven to have a lower surgical mortality and morbidity as well as a lower blood loss, a shorter hospital stay, and a better cosmetic result compared to the open procedure. This has resulted in LDN being considered the standard in many centers. Robot-Assisted Laparoscopic Donor (RALD) nephrectomy is a new trend developed in the last decade. Robotic assistance is increasingly popular worldwide, because it offers optimal operative conditions to the urological surgeon and a shorter learning curve than the standard laparoscopy.

Book Laparoscopic Donor Nephrectomy

Download or read book Laparoscopic Donor Nephrectomy written by Mahesh R. Desai and published by Springer. This book was released on 2017-03-30 with total page 182 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book explains the development of various laparoscopic donor nephrectomy (LDN) techniques, which can be performed via the retroperitoneal or the transperitoneal approach. It provides a step-by-step explanation of LDN, including details of intricacies such as port positioning, dissection steps, securing the hilum and retrieval. It also reviews new techniques like robotic donor nephrectomy and single-port donor nephrectomy. Lastly, it explores the non-technical but equally important issues of donor work-up and legal and social aspects. LDN is a unique surgical procedure in which the surgeon operates on an individual who is not a patient but someone donating out of altruistic motives, and it has to be “zero error” for the safety of the graft, donor and the recipient. This handbook is a valuable resource for urologists, general and transplant surgeons as well as anesthesiologists and medical social workers working in the field.

Living donor kidney transplantation is established as the best treatment option for kidney failure. However, despite the tremendous benefits of living donation to recipients and society, the outcomes and optimal care of donors themselves have received relatively less attention. Fortunately, things are changing – including recent landmark developments in living donor risk assessment, policy and guidance. This volume offers authoritative, evidence-based guidance on the full range of clinical scenarios encountered in the evaluation and care of living kidney donors. The approach to key elements of risk assessment, ethical considerations and informed consent is accompanied by recommendations for patient-centered care before, during, and after donation. Advocacy initiatives and policies to remove disincentives to donation and advance a defensible system of practice are also discussed. General and transplant nephrologists, as well as related allied health professionals, can look to this book as a comprehensive resource addressing contemporary clinical topics in the practice of living kidney donation.

Book A Report of the First 3 Cases of Robotic assisted Laparoscopic Kidney Transplantation in the UK

Download or read book A Report of the First 3 Cases of Robotic assisted Laparoscopic Kidney Transplantation in the UK written by Raphael Uwechue and published by . This book was released on 2017 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Introduction: Robotic assisted surgery has demonstrated clear benefits in other fields of abdominal surgery. We report the UK first experience of robot assisted kidney transplantation (RAKT).Methods: Clinical details of patients selected for living donor robotic transplantation were recorded. Primary outcome was renal function (eGFR) at one month, secondary outcomes included pain, opiate usage, length of stay and complications. Results: Case 1: 58 year old male with adult polycystic kidney disease and pre-dialysis. He had a BMI of 28. He had immediate graft function with a creatinine of 104uf06dmol/L one week after transplantation. He suffered a superficial wound infection within a month of surgery.Case 2: 42 year old female with anti-GBM disease and a failing first kidney transplant due to infections. She had a BMI of 29. She underwent RAKT to the left side. She had immediate graft function. Her creatinine on the seventh post-operative day was 125uf06dmol/L.Case 3: 49 year old female with chronic kidney disease from renal calculi who was a pre-emptive transplant. BMI of 26. She had immediate graft function. Her creatinine 1 month after surgery was 95u00b5mol/L.CasetRewarm time (mins)teGFR pre-opteGFR 1 MonthtMean pain score (0-10)tTotal morphine (mg)tDischarge day1t87t13t65t1t15t42t73t7t42t1.5t90t43t78t11t52t2.9t26t4Discussion: We have demonstrated the applicability and safety of this novel operative technique in the UK with early cases having significant potential in facilitating earlier discharge from hospital with less pain and a potentially superior post-operative recovery phase. The effect of the longer rewarming time needs to be evaluated.
