Primarily through the powerful tools of molecular biology, unified hypotheses explaining the origins of the disease are emerging and rapidly being validated. This volume, which presents the latest findings from laboratories throughout the world on the role of RNA tumor viruses in cancer, is a celebration of these achievements and a prediction of further progress leading ultimately to the control of the disease. It is important in this context to recall the natural history or life cycle of RNA cancer virology. From the earliest days of the science, when viruses were first recognized as distinct biologic agents of etiologic significance, their role in cancer was proposed and hotly debated. The critical early discoveries, even those made as recently as 25 years ago, were met with rejection; not skepticism or cautious restraint, but outright rejection. During the 60's, there was a gradual acceptance of the association between viruses and cancer, the result of landmark studies in experimental systems, and this led to a frenzy of activity in the field. There followed another period of doubt and uncertainty, due to the difficulty in attempting to apply directly, and in retrospect inappropriately, the tenets of infectious disease to human cancers, only to have the field resurrected, revitalized and redirected by the explosion of progress in molecular biology and genetics.

Book excerpt: The development of monoclonal antibodies to human tumor associated antigens has greatly facilitated the application of immunohistochemical techniques to analyze surgically removed tissues. During the last few years this approach has been utilized by a progressively increasing number of investigators to analyze malignant cells. Although monoclonal antibodies to tumor associated antigens have not become yet routine reagents in immunopathology, they have provided new information which could not be obtained with conventional antisera or histochemical procedures. The following are representative examples. TUmor associated antigens have been identified which display a restricted distribution in normal tissues and therefore may represent useful markers for radio imaging and appropriate targets for immunotherapy. In spite of undetec table differences with conventional histopathological approaches hetero geneity has been found in the antigenic profile of tumor cells within a lesion, in autologous lesions removed from different anatomic sites from a given patient and in lesions removed from different patients. Phenotypes of tumor cells have been identified which correlate with the biology of tumor cells and with the clinical course of the disease. From a practical view point the use of monoclonal antibodies in immunopathology has enhanced interactions between pathologists and immunologists, as exemplified by the present book. Such interactions have contributed to the application of basic research to clinical problems. The chapter of this book discuss investigations performed with monoclonal antibodies to antigens expressed by various types of normal and malignant human cells.

Book excerpt: The last 30 years have seen little improvement in the age-adjusted mortality rates for most common types of cancer, and until we develop more effective and less damaging treatment modalities for these tumours, selection of each patient's treatment must depend on prognostic pointers. These lead to a calculated trade off between our estimate of likely benefit to the patient, as against cost in terms of quality of life. But changes have occurred recently in our understanding of the traditional prognostic pointers used for selecting such individualised treatment. First, it is increasingly recognised that the stage at which a tumour presents is more related to the chromo logical age of the tumour (how far it has progressed before diagnosis) than to its biological characteristics. While advanced chronological age of the tumour may predict a greater likelihood of early death, only biological criteria can predict the tumour growth rate, the likelihood of prolonged survival, the likely course of the disease after the first recurrence or the likehood of response to systemic therapy. Second, there is increasing use of failure analysis in relating the clinical and biological characteristics of tumours to their response to standard treatments. In the past, the relationship was interpreted mainly in terms of survival rate, but the site and timing of first recurrence and the pattern and timing of subsequent spread provide a better assessment of the control possible from local or systemic therapy.

Conclusions regarding the efficacy of a particular modification of the therapeutic strategy can be strongly influenced by the assumed natural history of the tumor. I have tried to develop as accurateJy as the literature would allow a picture of the natural history of the common malignant solid tumors, knowing that the image would be imperfect. I adopted a convention which was employed in all graphs constructeil from case reports summarized from the literature.

Book excerpt: The addition of chemotherapy as an effective means to treat cancer has had a major impact on selected human malignancies. Due to a general inability to dif ferentiate between normal and neoplastic cells, little selectivity exists in currently used oncolytic drugs. Consequently, significant toxicity to the patient is expected when systemic cancer chemotherapy is chosen as an appropriate therapeutic in tervention. Much of this toxicity, such as damage to the bone marrow, gastroin testinal tract, or hair follicles, is predictable based upon the fact that anticancer drugs kill actively dividing cells. These types of toxicities, while serious, are usually manageable and reversible and are, therefore, not often considered to be dose limiting. Unfortunately, several of the most important anticancer drugs also damage tissues in which the growth fraction is relatively small. Such toxicities can not be predicted based on the chemical structure of the drugs, are often not detected in preclinical studies, and are encountered frequently for the first time in clinical studies. Further, unlike most of the proliferative-dependent toxicities, the unpre dicted toxicities are usually irreversible or only partially reversible upon cessation of drug administration. Because of this, the unpredicted toxicities are referred to as dose limiting. They represent a significant barrier to the ultimate efficacy of several of our most important anticancer drugs.

During the two-day conference, the major themes of brain oncology studies, both fundamental and clinical, were examined: oncogenesis, tumoral markers and immunology, metabolic and diagnosis imaging, prognostic factors and therapeutic strategies. The large number and high quality of participations resulted in a genuinely synthetic view of current advances in research, of which this book presents the essentials. We have attempted both to preserve the richness of scientific exchanges which occurred and to publish a great many oral and poster communications. The book respects the organization of conference sessions, and thus reflects the importance accorded to glial tumor studies. This may seem disproportionate given their frequency of occurrence in proportion to total numbers of intracranial tumor processes, but their gravity and quasi-total resistance to current therapeutic methods are ample justification. Moreover, this is the domain in which hopes of progress are beginning to appear, and it is becoming possible to envisage treatment based on recently established fundamental knowledge.
