Elle se définit par une perturbation des mécanismes de contrôle de la réaction immunitaire normale aboutissant à une réaction exagérée de l'organisme. Le chirurgien-dentiste, au même titre que son équipe, est confronté quotidiennement à des allergènes du fait de la manipulation de nombreux produits potentiellement allergisants, ce qui peut avoir un impact sur sa pratique professionnelle. Dans une première partie, nous rappellerons les mécanismes immunologiques de la réaction allergique, puis nous détaillerons les différentes substances présentes dans l'environnement de travail pouvant constituer des facteurs de risque : le latex, qui est un composant des gants, les produits désinfectants, détergents, antiseptiques utilisés quotidiennement pour l'hygiène des mains, des instruments et des surfaces, la manipulation répétée de résines (le méthacrylate de méthyle) et de métaux (nickel, titane). Dans une seconde partie, nous aborderons les différentes pathologies professionnelles dues au contact avec des allergènes en portant une attention plus particulière aux manifestations cutanées et respiratoires. En effet, un grand nombre d'allergènes au cabinet dentaire sont responsables de dermatites de contact et d'atteintes respiratoires comme l'asthme allergique. On observe également, dans de plus rares cas, des manifestations allergiques au niveau oculaire. Dans une troisième partie, nous exposerons les différentes possibilités de prise en charge de ces pathologies et insisterons sur l'importance des mesures de prévention préconisables pour la pratique professionnelle. Enfin, dans une dernière partie, nous illustrerons notre propos par les résultats d'une enquête réalisée dans une population active de chirurgiens-dentistes exerçant en milieu libéral ou en milieu hospitalier en France. La conclusion mettra en évidence l'importance de la mise en place de mesures de prévention ou de substituts de produits permettant au chirurgien-dentiste et à son équipe de minimiser le risque de maladie professionnelle dû aux allergènes et de pouvoir pratiquer dans les meilleurs conditions possibles.
