Book excerpt: The U.S. Global Change Research Program (USGCRP) was established in 1990 to "assist the Nation and the world to understand, assess, predict, and respond to human-induced and natural processes of global change."1 A key responsibility for the program is to conduct National Climate Assessments (NCAs) every 4 years.2 These assessments are intended to inform the nation about "observed changes in climate, the current status of the climate, and anticipated trends for the future." The USGCRP hopes that government entities from federal agencies to small municipalities, citizens, communities, and businesses will rely on these assessments of climate- related risks for planning and decision-making. The third NCA (NCA3) was published in 2014 and work on the fourth is beginning. The USGCRP asked the Board on Environmental Change and Society of the National Academies of Sciences, Engineering, and Medicine to conduct a workshop to explore ways to frame the NCA4 and subsequent NCA reports in terms of risks to society. The workshop was intended to collect experienced views on how to characterize and communicate information about climate-related hazards, risks, and opportunities that will support decision makers in their efforts to reduce greenhouse gas emissions, reduce vulnerability to likely changes in climate, and increase resilience to those changes. Characterizing Risk in Climate Change Assessments summarizes the presentations and discussions from the workshop.

Even if policy risk might seem lower today than a few years ago, political will can change quickly. Should physical impacts of climate change manifest in dramatic ways, for example, draconian climate policy is likely to follow quickly. These conditions create a complex and shifting business risk environment, and most companies either overlook or substantially underestimate key climate risks. How many companies, for example, are positioned for material climate change outcomes, whether physical or regulatory? Companies with little climate change exposure may not face much downside risk from taking a wait-and-see approach. For those with greater exposure, being "too late" to respond will mean costs and competitive impacts that could have been avoided. Being "too early," however, can mean being penalized later for actions that reduce a company’s emissions today, or competitive disadvantage from getting too far out in front of competitors.

Book excerpt: Alcohol is the sixth leading risk factor for disability and premature death all over the world, and one of the leading causes of premature mortality in western societies; it is a leading risk factor for death in young and middle-age males. Heavy drinking accounts for about two thirds of the burden of disease attributable to alcohol. In the early 1980s, screening and brief interventions (SBI) in primary health care settings were proposed as effective strategies to identify risky drinkers and to help them reduce their drinking. Since then, a growing body of evidence, including several meta-analysis and Cochrane reviews, has shown the efficacy and effectiveness of SBI in primary health settings. However, demonstrating the effectiveness of SBI has not been insufficient to facilitate its general implementation in the routines of primary health care physicians, and in fact the dissemination of SBI has proven to be a difficult business. Qualitative and quantitative research has identified most of the facilitators and barriers for its implementation, and publicly funded research has been earmarked to address the dissemination problems worldwide. Some examples are the World Health Organization Phase III and Phase IV studies on the identification and management of alcohol-related problems in primary care, EU funded projects (PHEPA, AMPHORA, ODHIN, BISTAIRS), the UK SIPS trials and the SBIRT developments sponsored by the Substance Abuse & Mental Health Services Administration (SAMHSA) in the USA. The efficacy and effectiveness of SBI in primary health is now well established, but there are still some questions that remain unsolved: which practitioners should deliver them; what length should they be; is there a need for booster sessions; is there added value of a motivational approach? These questions, together with other relevant aspects of SBI, need ongoing research. In recent years, SBIs have been tested in settings other than primary health care, including hospitals, accident and emergency rooms, criminal justice, colleges and universities, social services and pharmacies. In some of those areas, the evidence is scarce (for example, pharmacies) while in others it is very promising (for example, students and hospitals). New technologies have also offered the possibility of online tools, and, in the last few years, different digital-based applications have been tested successfully as new ways to deliver effective SBIs to larger amounts of people. Brief interventions have also spread to drugs other than alcohol. This book aims to be an update of the state-of-the art of brief advice. It is a compilation of articles published by some of the most relevant researchers in the field in Frontiers in Psychiatry between 2014 and 2016.

Subsequently, Kluwer Academic Publishers, evaluated an edited volume proposal package, and final revisions were made prior to submitting the entire manuscript for publication. Book excerpt: Risky Rhetoric: AIDS and the Cultural Practices of HIV Testing is the first book-length study of the rhetoric inherent in and surrounding HIV testing. In addition to providing a history of HIV testing in the United States from 1985 to the present, J. Blake Scott explains how faulty arguments about testing’s power and effects have promoted unresponsive and even dangerous testing practices for so-called normal subjects as well as those deemed risky. Drawing on classical rhetoric as well as Michel Foucault’s theorizing of the examination as a form of disciplinary power, this study explores how HIV testing functions as a disciplinary technology that shapes subjects and exerts power over individual bodies and populations. Testing has largely been deployed to protect those defined as normal members of the general population by detecting, managing, and even punishing those diagnosed as risky (e.g., gay and bisexual men, poor women of color). But Scott reveals that testing’s function of protection-through-detection has been fueled in part by faulty arguments that exaggerate testing’s interventive power and benefits. These arguments have also created a perception that testing is a magic bullet. By overestimating the benefits of HIV testing and overlooking its contingencies and harmful effects, dominant arguments about testing have enabled a shortsighted public health response to HIV and unresponsive testing policies. The ultimate goal of Risky Rhetoric: AIDS and the Cultural Practices of HIV Testing is to offer strategies to policymakers, HIV educators and test counselors, and other rhetors for developing more responsive and egalitarian testing-related rhetorics and practices.
