Available in PDF, EPUB and Kindle. Book excerpt: People migrate to improve their well-being, whether through an expansion of economic and social opportunities or a reduction in persecution. Yet a large literature suggests that migration can be a stressful process, with potentially negative impacts on mental health, reducing the net benefits of migration. However, to truly understand the effect of migration on mental health one must compare the mental health of migrants to what their mental health would have been had they stayed in their home country. The existing literature is not able to do this and typically settles for comparing the mental health of migrants to that of natives in the destination country, which takes no account of any preexisting differences between these groups. This paper overcomes the selection problems afffecting previous studies of the effect of migration on mental health by examining a migrant lottery program. New Zealand allows a quota of Tongans to immigrate each year with a lottery used to choose amony the excess number of applicants. A unique survey conducted by the authors in these two countries allows experimental estimates of mental health of migrants who were successful applicants in the lottery to the mental health of those who applied to migrate under the quota, but whose names were not drawn in the lottery. Migration is found to lead to improvements in mental health, particularly for women and those with poor mental health in their home country.

The focus on common health needs of refugee and migrant populations across the life-course is designed to complement the development of the Global Competency Standards for the effective provision of health services to refugees and migrants. Health workers providing care to refugees and migrants should be aware of how the migration experience – which can involve poor transit conditions, restrictive entry and integration policies, exclusion and acculturation stress – influences the health status of individuals and their health needs.

Book excerpt: The increasing number of refugees, asylum seekers, and irregular migrants poses a challenge for mental health services in Europe. This review found that these groups are exposed to risk factors for mental disorders before, during, and after migration. The prevalence of psychotic, mood, and substance-use disorders in these groups varies but overall resembles that in the host populations. Refugees and asylum seekers, however, have higher rates of post-traumatic stress disorder. Poor socioeconomic conditions are associated with increased rates of depression five years after resettlement. Refugees, asylum seekers, and irregular migrants encounter barriers to accessing mental health care. Good practice for mental health care includes promoting social integration, developing outreach services, coordinating health care, providing information on entitlements and available services, and training professionals to work with these groups. These actions require resources and organizational flexibility.

Yet suicide, heavy drinking, violence, family separations, and domestic disharmony were but a few of the possible struggles experienced by those who relocated abroad in the nineteenth and twentieth centuries, and were among the chief reasons for committal to an asylum. Significant analysis of this problem, addressing the interconnected issues of migration, ethnicity, and insanity, has to date received little attention from the scholarly community. This international collection examines the difficulties that migrants faced in adjustment abroad, through a focus on migrants and mobile peoples, issues of ethnicity, and the impact of migration on the mental health of refugees. It further extends the migration paradigm beyond patients to incorporate the international exchange of medical ideas and institutional practices, and the recruitment of a medical workforce. These issues are explored through case studies which utilize different social and cultural historical methods, but with a shared twin purpose: to uncover the related histories of migration, ethnicity, and mental health, and to extend existing scholarly frameworks and findings in this under-developed field of inquiry.

The use of grounded theory (GT) was used to systematically identify and select appropriate articles. The articles were screened through inclusion and exclusion criteria and were coded for themes. Four themes were identified along with sub-themes, which were: Theme 1. Mental Health Occurrences, with the sub-themes of Depression, Anxiety, and PTSD in Adults, and Depression, Anxiety, and PTSD in Youth; Theme 2. Impact of Timing on Mental Health, with the sub-themes of Pre-migration and Post-migration; Theme 3. Protective Factors, with the sub-themes of Social Ties and Healthcare Providers; and, Theme 4. Access to Care, with sub-themes of Barriers, Facilitators, and Solutions. Future research should focus on addressing health inequities.
