Book excerpt: This book investigates death after police contact in England and Wales in the twenty-first century. It examines how regulatory bodies construct accountability in such cases. Cases of death after police contact have the potential to cause deep unease in society. They highlight the unique role of the police in being legitimately able to use force whilst at the same time being expected to preserve life. People who are from Black, or Minority Ethnic backgrounds, or have mental health issues, or are dependent on substances are disproportionately more likely to die in these cases, and this emphasises the sensitive nature of many of these deaths to society. Deaths after Police Contact examines police legitimacy and the legitimacy of police regulators in these cases. The book argues that accountability is produced by a relatively arbitrary system of regulation that investigates such deaths as individual cases, rather than attempting to learn lessons from annual trends and patterns that might prevent future deaths. It will be of great interest to scholars and upper-level students of policing and criminal justice.

Despite the large range of institutions that monitor English and Welsh prisons, suicides reached a record high in 2016, with the rate having doubled since 2012. These deaths represent the sharp end of a continuum of suffering, self-harm, despair and distress within prisons, which affects prisoners, their families and prison staff. This book details and critiques the lengthy and expensive police, ombudsman and coroner investigations that follow prison suicides. Drawing on extensive document analysis, including analysis of over 100 Prison and Probation Ombudsman fatal incident investigations, and original semi-structured interviews with stakeholders undertaken between 2016-2017, this book provides a novel analysis of prison oversight.

Though practice at that time was generally not breached it was unsatisfactory and that 'there was systemic failure to protect Sonny Lodge in the last days of his life..'

This second reply addresses the most significant recommendation of the Report, that a cross-departmental expert task force should be set up to look at deaths in all forms of state custody. The Government describes a number of institutions and points to the establishment of the multi-agency group chaired by the Independent Police Complaints Commission. However the Committee has concerns that the need for an active, interventionist body has not been met. They will therefore keep the matter under active review in the current Parliament.

Book excerpt: Although road accident deaths have halved between 1958 and 2007 whilst the number of licensed motor vehicles and vehicle mileage covered increased by 400 per cent, the current rate of 3,000 deaths and 250,000 injuries is still an unacceptably high level. Road accidents are the largest single cause of death for people between the ages of 5 and 35 in Britain, and road accidents cost our economy some £18 billion each year. The number of deaths and injuries on roads far outweighs the deaths and injuries in other transport modes, and should be viewed as a major public health problem. The Government should establish a British Road Safety Survey to track overall casualty and safety trends, and review current methods for recording road-traffic injuries. The Committee recommends a systems approach to road safety: ensuring the vehicle, the road infrastructure, regulations and driver training are designed to similar safety and performance standards. Other recommendations include: more 20 mph speed limits; a more proactive approach to determining the safety benefits of new vehicle technologies; action on young drivers - who represent a disproportionate risk to road users - and vulnerable users: motorcyclists, elderly and child pedestrians and cyclists, horse riders; a higher priority given to enforcement of drink-drive and drug-drive offences. The Committee recommends the establishment of an independent Road Safety Commission with powers to work across the whole of government, ensuring that a high priority and adequate resources are given to road safety and that all government departments and agencies give active support. The Government should also establish a road accident investigation branch, like those in aviation, rail and marine.

Book excerpt: Public authorities have a duty to ensure looked after children are not at greater risk of being drawn into the criminal justice system than other children. The relevant authorities must continue to support looked after children and care leavers when they are in, and when they leave, custody. The substantial decrease since 2006/07 in the number of young people entering the criminal justice system for the first time is welcomed but looked after children have not benefited from this shift to the same extent as other children. The Youth Justice Board has done excellent work to halve the youth custodial population over the past decade but continues to spend £246 million a year detaining a small fraction of young offenders. Recommendations include: a statutory threshold to enshrine in legislation the principle that only the most serious and prolific young offenders should be placed in custody; devolving the custody budget to enable local authorities to invest in effective alternatives to custody; and more action to reduce the number of young people who breach the terms of their community sentences and the number of young black men in custody. The aim of improving the basic literacy of offenders, as outlined in the Transforming Youth Custody consultation paper is endorsed, but is it most useful to focus resources on the secure estate, given that the average length of stay is currently 79 days? The greater focus should be on improving transition between custody and the community, and on improving provision in the community and incentivising schools and colleges to take back difficult students.
