Book excerpt: Dementia, a chronic or persistent disorder of the mental process caused by brain disease or injury, usually affects elderly people over 65 years and is characterized by memory loss and personality changes. Currently about 5.4 million Americans are affected by this condition. Dementia patients are often agitated, aggressive, wanderers and at high risk for falls. During hospitalization they get more agitated due to various reasons and in order to manage their behavior, they are often placed on restraints. Restraining has both physical and psychological impacts on patients. Fifteen evidence-based, peer-reviewed research articles from 2008-2013, which include quantitative, qualitative, longitudinal and cross-sectional articles were analyzed and appraised as part of this project and Kurt Lewin's change theory is incorporated. An implementation plan including identifying the problem, implementation logistics, and implementation resources are identified. As part of the evaluation plan, areas of improvement and variables are identified. Stakeholders as well as staff's current knowledge and attitude is evaluated by using organizational questionnaires. Educational pamphlet is developed to educate the participants, pre and post-tests are conducted. A dissemination strategy is developed as the final phase of the project to raise awareness, to educate the community, to get feedback and to promote and make sustainable results of the project. Based on evidence and documented studies, a patient-centered approach with meaningful interaction could minimize the use of restraints on dementia patients with aggressive behavior during their hospitalization. Staff education and skills development tools are important to implement this cost effective, patient-centered approach which could improve patient satisfaction and quality of care.

Patients with dementia, which stem from conditions such as, Alzheimer's, have a disorder that is progressive and makes caring for the patient difficult. These patients have trouble remembering, thinking clearly, concentrating or completing activities of daily living (Family Caregiver Alliance, 2015). Restraints are a method used to control unsafe patient behavior, which could lead to patient falls. Examples of these behaviors include wandering, unsteady gait, and agitated or aggressive behaviors. Restraint use, however, remains controversial. There are also many adverse effects which patients suffer as a result of restraint use, up to and including death. Research regarding the use of environmental adaptations shows promise. The environment when defined should consist of the patients external and internal surroundings. This means that nurses should assess the patient for symptoms of pain, infection, reaction to medications and other imbalances which could affect their internal environment. The external environmental should be assessed and modified to suit patient's safety needs. Recommended for external environmental modification include increased lighting, non-slip floors, individual seating, locked wheelchairs, bed stabilizers, bedside commodes at night, room near the nursing station, and electronic warning devices (Tse, 2005).
