Book excerpt: Mr F complained that: The Department for International Development (DFID) failed to conduct a thorough and independent investigation into his allegations, or exercise reasonable oversight of CDC Group plc (CDC); and DFID did not appropriately handle the allegations and information that Mr F sent. Mr F's allegations concerned inappropriate use of funds by one of the fund managers in whom CDC invested. He says he has been caused outrage as a result of DFID's actions and says that because DFID did not secure his anonymity he has suffered threats and has been unable to return to his life in Nigeria. We partly uphold Mr F's complaint. PHSO found maladministration in the way DFID investigated and reported on its consideration of Mr F's allegations. However, it was not found that DFID was unreasonable in its conclusions. DFID did not ensure Mr F's confidentiality, but this did not lead to all the injustice Mr F claims

This book was released on 2011-09-14 with total page 108 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report concerns a complaint by Mr A (now deceased) and his siblings, who were British civilians interned by the Japanese in Singapore in 1945. In 2000 they applied to the compensation scheme set up by the British Government to recognise the 'debt of honour' owed by the UK to British prisoners of war and civilian internees. They were initially denied compensation because they did not have a close enough link to the UK to qualify, but received a £500 payment and an apology following the Ombudsman's intervention. In 2007, the MoD set up a further scheme to compensate those whose applications to the original scheme were wrongly rejected. Mr A's family was invited to apply to this second scheme, but their application was refused and they were told that the previous apology and payment had been given to them in error. The investigation found that Mr A and his siblings were subjected to prolonged and aggravated distress by the British Government during the 10 years that they struggled to resolve their compensation claims with the MoD. The MoD mismanaged the administration of the second compensation scheme and had incorrectly and offensively retracted a previous apology issued to them. The Secretary of State for Defence should apologise personally to the family and pay them the compensation wrongly denied to them (£4,000 each) plus a further £5,000 each in recognition of the distress they suffered. The MoD has accepted all the recommendations and will launch its own review of what went wrong

Book excerpt: This report deals with a complaint concerning personal address details which were incorrectly changed by an unidentified government agency and then duplicated across other government agencies with whom the complainer was involved. Furthermore the error was not corrected once advised. None of the bodies in question had been able to explain satisfactorily what had gone wrong and none had, therefore, resolved the complaint. The legislation concerned is the Data Protection Act 1998 which regulates how personal information should be processed, including how data is obtained, held, used and disclosed. The Parliamentary Ombudsman investigated and upheld the complaint. It was concluded that the individual and collective failings amounted to both maladministration and a failure of public service. Recommendations were made and agreed in full
