Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Ours is "intended to endure for ages to come, and consequently, to be adapted to the various crises of human affairs." In recent years, Marshall's great truths have been challenged by proponents of originalism and strict construction. Such legal thinkers as Supreme Court Justice Antonin Scalia argue that the Constitution must be construed and applied as it was when the Framers wrote it. In Keeping Faith with the Constitution, three legal authorities make the case for Marshall's vision. Lacking trained military engineers, the Americans relied heavily on foreign officers, mostly from France, for sorely needed technical assistance. Native Americans joined the foreign engineer officers to plan and carry out offensive and defensive operations, direct the erection of fortifications, map vital terrain, and lay out encampments. During the war Congress created the Corps of Engineers with three companies of engineer troops as well as a separate geographer's department to assist the engineers with mapping. Both General George Washington and Major General Louis Lebéque Duportail, his third and longest serving Chief Engineer, recognized the disadvantages of relying on foreign powers to fill the Army's crucial need for engineers. America, they contended, must train its own engineers for the future. Accordingly, at the war's end, they suggested maintaining a peacetime engineering establishment and creating a military academy. However, Congress rejected the proposals, and the Corps of Engineers and its companies of sappers and miners mustered out of service. Eleven years passed before Congress authorized a new establishment, the Corps of Artillerists and Engineers.
