La consommation répétée de cannabis entraine des conséquences physiques et psychologiques au long cours. Le médecin généraliste occupe une place centrale dans le repérage de la consommation. La consultation avec l'adolescent de par ses spécificités de communication est encore trop souvent une consultation manquée où le repérage n'est pas pratiqué. Le but de l'étude était d'évaluer comment faciliter l'abord et le repérage du cannabis lors des consultations avec les adolescents en médecine générale. Étude qualitative, menée par deux investigatrices. Réalisation de 17 entretiens semi dirigés auprès de médecins généralistes. Les entretiens ont été enregistrés, retranscrits, anonymisés puis analysés avec respect de la triangulation. Le travail de recherche a été mené selon une méthode d'analyse thématique. Le médecin peut aborder le sujet du cannabis de façon directe ou indirecte s'il repère des signes d'alerte. Pour faciliter le repérage le médecin généraliste doit adapter son mode de communication à l'adolescent et réussir à instaurer un climat de confiance. Le médecin généraliste doit faire preuve d'habileté communicationnelle, adopter un discours ouvert, sans jugement ni a priori. Il importe de rappeler aux adolescents, à leurs parents et aux médecins généraliste que le jeune a un droit au secret médical. Il importe d'éduquer parents et jeunes enfants à l'importance d'un suivi régulier pendant l'adolescence avec le médecin traitant.

Introduction : La France est un des pays d'Europe qui compte le plus de consommateurs de cannabis. La dangerosité de la consommation de cannabis dès le plus jeune âge, et ses répercussions médico-psycho-sociales à court et long termes nécessitent de repérer et de prendre en charge précocement ces consommateurs. Malgré l'existence d'outils validés, de nombreux freins au repérage précoce-intervention brève (RPIB) ont été décrits dans diverses études. Objectif : Analyser les motivations de médecins généralistes sensibilisés à l'addictologie et membres d'un réseau d'addictologie afin de comprendre les déterminants d'appropriation des outils de RPIB chez l'adolescent consommateur de cannabis, et l'intérêt de faire partie d'un réseau. Comprendre ce qui permettrait de lever les obstacles à la pratique de ce repérage. Matériels et méthodes : Etude qualitative réalisée en avril 2018 auprès de 8 médecins généralistes libéraux du Vaucluse et du Gard formés en addictologie et membres d'un réseau d'addictologie, par entretien semi-dirigé, en focus group ou entretiens individuels. Résultats : Ces médecins se sont formés en addictologie et ont adhéré au réseau car ils avaient des lacunes de connaissances tout en étant confrontés à des patients consommateurs de substances psychoactives en demande de soins ; ils souhaitaient améliorer leur prise en charge. Plusieurs ont assisté à l'émergence de l'addictologie lors de l'épidémie du VIH, et ont été pionniers des prescriptions de TSO, vécues comme des actes de militantisme. La formation leur a apporté un regard nouveau sur les usagers de cannabis, des connaissances scientifiques et actualisées sur le sujet, et des outils de consultation. Ils sont motivés pour faire du RPIB car ils considèrent que cela fait partie de leur rôle de médecin traitant, et ils se sentent à l'aise grâce à leurs connaissances sur la substance et les moyens de prise en charge. Les outils de repérage qu'ils utilisent sont l'outil relationnel, différentes techniques d'entretien, un repérage systématique ou ciblé devant des symptômes ou des situations qui interpellent. Aucun n'utilise les questionnaires de repérage jugés inappropriés à leur pratique. Pour prendre en charge, ils pratiquent une intervention brève ou orientent vers une structure spécialisée si nécessaire. Si certains freins au repérage persistent, ils arrivent néanmoins à s'adapter au ressenti du manque de temps, à la présence parentale, et au rapport à l'adolescent. Conclusion : Nombre d'outils de repérage de l'usage de cannabis chez l'adolescent sont applicables à la pratique du médecin généraliste s'il les possède. Il est important de former en nombre les généralistes afin d'améliorer la prise en charge de ces patients.

Le but de cette étude est d’explorer les méthodes de dépistage clinique de l’usage de cannabis chez les adolescents en médecine générale. Méthode : Une étude qualitative a été réalisée par entretiens individuels semi-directifs auprès de treize médecins généralistes dans l’Hérault de juin à octobre 2015. Une analyse thématique a été effectuée à partir des données retranscrites. Résultats : L’usage de cannabis n’est pas abordé de manière systématique par les médecins généralistes chez tous les adolescents. Une fois dépisté, l’usage problématique est constamment recherché. Au final, deux profils sont évoqués : les consommateurs opportunistes et les consommateurs réguliers. Conclusion : Cette étude met en évidence l’importance du repérage systématique de la consommation de cannabis chez les adolescents. Une technique de communication adaptée est nécessaire pour créer une relation de confiance ce qui facilite l'abord de ce thème fondamental et complexe.

Book excerpt: Introduction : Le cannabis est le stupéfiant le plus largement consommé en France , surtout par les jeunes. 41,5% des jeunes de 17 ans ont expérimenté le cannabis (chiffres 2011). 60% es jeunes consommateurs ont plus de risque de ne pas terminer leurs études secondaires et de ne pas réussir l'examen final, par rapport à ceux qui n'ont jamais fumé. La consommation de cannabis est une problématique complexe et de plus en plus courante chez les adolescents. Le médecin généraliste a un rôle important en matière de prévention, en particulier face aux adolescents, il est d'ailleurs l'un des professionnels de première ligne à pouvoir le faire. Ce travail est né d'un questionnement personnel: comment un médecin généraliste peut venir en aide aux jeunes consommateurs de cannabis et quels sont les moyens mis à sa disposition pour assurer une prise en charge optimale? C'est dans ce sens qu'une enquête a été organisé auprès des médecins généralistes de Picardie pour voir comment ils abordent cette problématique. Méthode : Une étude descriptive a été réalisé auprès de 135 médecins généraliste de Picardie. leurs connaissances et pratiques ont été évalués par un questionnaire comprenant vingt-sept questions. Résultats : 3% des médecins interrogés recherchent systématiquement une consommation. Ils connaissent les risques du cannabis , mais leur prise en charge n'est pas optimale à cause de la méconnaissance des outils et des centres spécialisés. 83% déclarent nécessaire d'avoir une formation sur les addictions, mais très peu en sont demandeurs. Discussion : Améliorer la formation des médecins généralistes , les encourager à participer et à s'impliquer davantage dans le dépistage, semble impératif. Faire une diffusion plus importante des outils et modalités d'aide au repérage pourrait convaincre les médecins de prendre en charge les adolescents consommateurs. Conclusions : Les médecins généralistes connaissent les risques et les effets de la consommation de cannabis, mais ne sont pas suffisamment formés pour prendre en charge un adolescent consommateur de cannabis.

Evaluation de l'impact sur la pratique médicale à un an d'une formation. INTRODUCTION : Les médecins généralistes (MG) sont peu nombreux à rechercher une consommation de cannabis chez leurs jeunes patients. La principale raison évoquée est le manque de formation sur le cannabis. Il paraissait intéressant de proposer une formation à des MG picards puis d'évaluer son impact un an après sur la pratique médicale. METHODE : Une formation a été élaborée puis proposée à des MG en novembre 2014. Ces médecins ont été revus un an plus tard en novembre 2015 afin de réaliser une étude qualitative par entretiens semi-dirigés. Ces entretiens ont été analysés selon la méthode de la théorisation ancrée. RESULTATS : Dix entretiens ont été réalisés. Un an après la formation, les médecins recherchaient plus fréquemment et avec plus d'aisance une consommation de cannabis auprès de leurs patients. Ils remettaient plus facilement en question leurs pratiques concernant le dépistage du cannabis. Les MG présents ont apprécié la formation, particulièrement la projection de vidéos de consultation mettant en scène la recherche du cannabis. DISCUSSION : Des solutions ont été proposées pour améliorer l'impact des formations proposées aux MG sur le thème du cannabis. Organiser des formations interactives et répétées dans le temps semble être la méthode la plus efficace et permettrait d'aider à lever certains freins au dépistage du cannabis en consultation. CONCLUSION : La formation a eu un impact sur les pratiques des MG présents. Il pourrait être intéressant de continuer dans ce sens avec des stratégies de formation sur le dépistage du cannabis à plus grande échelle afin de toucher un plus grand nombre de médecins.

Neorealism, the traditional story goes, was an Italian film style born in the second postwar period and aimed at recovering the reality of Italy after the sugarcoated moving images of fascism. Lasting from 1945 to the early 1950s, neorealism produced world-renowned masterpieces such as Roberto Rossellini's Roma, città aperta (Rome, Open City, 1945) and Vittorio De Sica's Ladri di biciclette (Bicycle Thieves, 1947). These films won some of the most prestigious film awards of the immediate postwar period and influenced world cinema. This collection brings together distinguished film scholars and cultural historians to complicate this nation-based approach to the history of neorealism. The traditional story notwithstanding, the meaning and the origins of the term are problematic. What does neorealism really mean, and how Italian is it? Italian filmmakers were wary of using the term and Rossellini preferred "realism." Many filmmakers confessed to having greatly borrowed from other cinemas, including French, Soviet, and American. Divided into three sections, Global Neorealism examines the history of this film style from the 1930s to the 1970s using a global and international perspective. The first section examines the origins of neorealism in the international debate about realist esthetics in the 1930s. The second section discusses how this debate about realism was “Italianized” and coalesced into Italian “neorealism” and explores how critics and film distributors participated in coining the term. Finally, the third section looks at neorealism’s success outside of Italy and examines how film cultures in Latin America, Africa, Asia, and the United States adjusted the style to their national and regional situations.

Do these attempts at self-regulation work? In this book, Tijana Milosevic examines the effectiveness of efforts by social media companies—including Facebook, Twitter, YouTube, Snapchat, and Instagram—to rein in cyberbullying by young users. Milosevic analyzes the anti-bullying policies of fourteen major social media companies, as recorded in companies' corporate documents, draws on interviews with company representatives and e-safety experts, and details the roles of nongovernmental organizations examining their ability to provide critical independent advice. She draws attention to lack of transparency in how companies handle bullying cases, emphasizing the need for a continuous independent evaluation of effectiveness of companies' mechanisms, especially from children's perspective. Milosevic argues that cyberbullying should be viewed in the context of children's rights and as part of the larger social problem of the culture of humiliation. Milosevic looks into five digital bullying cases related to suicides, examining the pressures on the social media companies involved, the nature of the public discussion, and subsequent government regulation that did not necessarily address the problem in a way that benefits children. She emphasizes the need not only for protection but also for participation and empowerment—for finding a way to protect the vulnerable while ensuring the child's right to participate in digital spaces.
