Book excerpt: Background: Implant stability has been recognized as an essential factor link to osseointegration and predicts the survival rate of dental implants. The resonance frequency analysis (RFA) is a noninvasive device providing results in the form of the implant stability quotient (ISQ). The primary stability is also influenced by numerous factors such as surgical technique, implant morphology as well as bone quality and quantity. In the posterior maxilla, the sparse bone density leads to low primary stability. Aim: This retrospective study aims to analyze the factors that influenced the primary stability of dental implant of posterior maxilla.Material and methods: Patients records of treatment performed between January 2014 to December 2018 were reviewed for primary implant stability. Patients who had Straumannu00ae implants at posterior maxilla and exhibited RFA both buccal and mesial area immediate implant placement were included. The following variables were determined; patient age, gender, implantation region, timing of implant placement after tooth extraction (immediate placement, u2264 4 months after extraction, > 4 months after extraction), implant geometry (diameter and length), and adjunctive procedures during implantation (guided bone regeneration and transcrestal osteotomy). The RFA value was used The Kolmogorov-Smirnov Test to determine normal distribution. The Chi-Square Test was applied to compare the relationship between qualitative variables and the Kruskalu2013Wallis test was used to compare the mean. The Pearson correlation coefficient was used to determine correlation.Results: Seventy implants in 53 patients (male 19, female 34) were found. The mean age of this sample size is 59.04uf0b11.41 years old. The mean value of RFA of the buccal area is 65.76uf0b11.44 and the mean of RFA of mesial area 66.59uf0b11.55. Book excerpt: Background & purpose:tImplant stability is one of the most important factors influencing osseointegration. Using stereolithographical guides for maximizing precision, this study aimed at investigating the relationship between implant stability and bone density derived from CT (Computerized Tomography) analysis.Materials & methods:195 implants were placed in 48 patients using digitally designed stereolithographical surgical guides. 95 implants were placed using a mucosa supported guide and 100 implants were placed using a bone supported guide. Implant stability was measured by means of Resonance Frequency Analysis (RFA) and damping capacity assessment (Periotest, PTV). Bone density (Hounsfield units) was measured at different ROI (regions of interest) and cortex thickness was measured around each implant.Results:Implant stability correlated significantly with the different ROI. The best correlation for RFA was obtained for the spongious bone ROI (r = 0.64) and PTV best correlated with the coronal cortex density (r = -0.41). Shorter implants (9 mm) had a significantly lower primary stability than longer implants (11, 13, 15 mm). Primary stability was also significantly higher in 4mm diameter implants than in 3.5mm diameter implants. A formula for the prediction of primary stability based on the different variables investigated was developed.Conclusions:tBone density and cortex thickness have a significant influence on implant primary stability. Longer and wider implants reached higher primary stability than shorter and narrower implants. These correlations lose their significance after osseointegration has taken place. Implant stability can be predicted based on an pre-operative analysis of bone characteristics.

Book excerpt: Background: Success and survival of dental implants are affected by the alveolar bone quality and bone mineral density (BMD) of the implant bed.These parameters are considered key factors for primary implant stability and influence the load-bearing capacity of the implant.Studies on the effect of bone quality and bone mineral density on the long-term outcome of dental implants are limited. A review wasinitiated to evaluate their effect on dental implant failures. Aim: A systematic review of the literature was conducted to evaluate the effect of bone quality and bone mineral density on early and lateimplant failures. Implant failures were defined as peri-implantitis and implant loss.Materials and Methods: A literature search was performed using the electronic databases PubMed/MEDLINE, EMBASE, Cochrane Library and Web of Science to identify studies published in English between 1990 and January 2018. Clinical trials reporting on patients who underwentdental implant placement at various locations in the mandible and maxilla were included. Bone quality and/or density had to be evaluated clinically, radiologically or with Dual-energy X-ray absorptiometry (DXA) before, during or after implant placement.Differences between healthy patients versus patients with impaired bone quality or bone mineral density due to systemic diseases were also compared.A bias and methodology quality assessment was performed using the Checklist for Health Care Interventions Studies as proposed byDowns and Black. The review protocol was registered at the PROSPERO database (number CRD42019122722).Results: In this review 16 non-randomized retrospective studies and 3 prospective cohort studies involving 5422 patients were included.Eleven studies reported on the outcome of implant therapy in relation to bone quality and BMD in healthy patients. Eight studies assessed the bone quality or mineral density in patients with bone-affecting systemic diseases. Due to the heterogeneity of the studies and outcome variables meta-analysis of the data was not possible.Peri-implantitis was an outcome measure in three studies. All included studies investigated implant loss. Lower BMD measurements and bone quality type 4 appear to be associated with lower implant stability and more early implant loss. No long-term data could be retrieved to evaluate the effect of bone quality and BMD on late implant failures. The studies that investigated patients with bone-affecting systemic diseases did not find a relationship between systemic BMD and local density of the alveolar bone. Conclusions: Dental implants in patients with bone-affecting systemic diseases are a predictable treatment option for oral rehabilitation. The included studies did not show a relationship between the systemic bone mineral density and the local density of the implant bed. Clinically thisimplies that in sites with low local bone mineral density, adaptation of the surgical technique and an extended submerged healing period are recommended.

Since primary stability is a direct consequence of bone-implant contact, it depends on bone density, surgical technique, and macroscopic and microscopic morphology of the implant used. Primary stability can be either measured by insertion torque or ressonance frequency analysis, but no relation between these two is clearly described. The objective of this study was to evaluate the statistical correlation of primary stability through ressonance frequency analisys (RFA) and manual torque of insertion through literature review and case series study.This study was conducted on a post-graduate clinic (CLIVO- Rio de Janeiro,Brazil), where fourteen dental implants had their manual torque and RFA measured using a manual torque device during instalation. RFA measured the Implant stability Quoeffcient(ISQ) using a smartpeg after instalation in 5 different positions ( bucal, lingual, occlusal,mesial and distal).All of the fourteen implants had the same conical connection and were placed in the mandible or maxila. The mean ISQ measurements and manual torque were submitted to statistical analysis to determine a possible correlation.We evaluated 13 implants, 9 implants performed in female patients and 4 male patients, 5 in maxilla and 8 in mandible. For the study, internal connical connection dental implants were used. In order to verify the correlation between TORQUE and ISQ, the statistical software.
