PMNs integrate opposing cues within the inflammatory microenvironment, which converge on selected receptors. Among these receptors is the pleiotropic receptor, formyl peptide receptor 2/lipoxin receptor (FPR2/ALX), which can recognize over 20 structurally diverse proteins, peptides, and lipid ligands. Among these ligands are N-formylated peptides released from mitochondria of dying or dead host cells. At present, little is known about the molecular basis for how FPR2/ALX integrates opposing cues and triggers diverse biological responses. We wanted to test the effect of N-formylated peptides on human neutrophils' functions in sepsis-like syndrome. We found that the pleiotropic receptor FPR2/ALX integrates opposing signals that govern phagocytosis, bacterial killing, the fate of neutrophils, and ultimately the outcome of the inflammatory response. This action of N-formylated peptides is mediated through induction of the release of neutrophil elastase from the primary granules and subsequent downregulation of complement C5a receptor on the cell surface. This effect can be reversed by pro-resolving lipid mediators, aspirin-triggered 15-epi-lipoxin A4 (15-epi-LXA4) and 17-epi-resolvin D1 (17-epi-RvD1). Our results could lead to the development of novel pharmacological strategies to enhance FPR2/ALX-mediated protective mechanisms that may limit the inflammatory response and promote the resolution of inflammation.

When I was a medical student, lectures in cytology were a special, segregated part of the curriculum in the histology course, given along with general anatomy, and they were, as I recall, the surest of cures for insomnia. I still possess Cowdry's three-volume set entitled Special Cytology, published in 1934, and leafing through these books today is rather like examining a medieval manuscript. You could never have guessed what was going to happen to the field. At that time it was all structure, and all guesswork about the structure. When cells were packed together in various tissues, how did the geometry of packing work? How many sides did a liver cell have, in real life? What on earth were all those granules in side, and what were the best stains for looking at them? One thing about those granules, they never moved. Indeed, nothing moved. Cytology turned into cell biology much later on, and suddenly came alive. As has been the case in so many diSciplines in biology, it was brought to life by techniques. New instruments and cytochemical methods were devised for look ing at cells, manipulating cells, more or less in vivo.

It is suggested that many diseases are initiated and their progress is influenced by inflammatory molecules and a decrease in the production and/or action of anti-inflammatory molecules and this imbalance between pro- and anti-inflammatory molecules seems to have been initiated in the perinatal period. This implies that strategies to prevent and manage various adult diseases should start in the perinatal period. An alteration in the metaolism of essential fatty acids and their anti-inflammatory molecules such as lipoxins, resolvins, protecitns, maresins and nitrolipids seems to play a major role in the pathobiology of several adult diseases. Based on these concepts, novel therapeutic approaches in the management of insulin resistance, obesity, type 2 diabetes mellitus, metabolic syndrome, cancer, lupus, rheumatoid arthritis and other auto-immune diseases are presented. Based on all these evidences, a unified concept that several adult diseases are due to an alteration in the balance between pro- and anti-inflammatory molecules is discussed and novel methods of their management are presented.
