Available in PDF, EPUB and Kindle. Book excerpt: Squamous cell carcinoma of the oral cavity (SCCOC) is one of the most prevalent tumors of the head and neck region. Despite improvements in treatment, the survival of patients with SCCOC has not significantly improved over the past several decades. Most frequently, treatment failure takes the form of local and regional recurrences, but as disease control in these areas improves, SCCOC treatment failures more commonly occur as distant metastasis. The presence of cervical lymph node metastasis is the most reliable adverse prognostic factor in patients with SCCOC, and extracapsular spread (ECS) of cervical lymph nodes metastasis is a particularly reliable predictor of regional and distant recurrence and death from disease. Decisions regarding elective and therapeutic management of cervical lymph node metastases are made mainly on clinical grounds as we cannot always predict cervical lymph node metastasis from the size and extent of invasion of the primary tumors. Therefore the treatment of the neck disease in the management of SCCOC remains controversial. The promise of using biomarker-based treatment decisions has yet to be fully realized due to our poor understanding of the mechanisms of regional and distant metastases of SCCOC. We will summarize the current status of investigations into SCCOC metastases and potential of these studies to impact basic research investigators and clinicians confronting SCCOC in the future.

The risk of occult metastasis is related to the method by which the lymph nodes are evaluated. It is possible to reduce the risk of undiagnosed metastasis with accurate imaging techniques and thus probably reduce the number of elective neck treatments. The aim of this study was to assess the accuracy of clinical palpation, CT Scan, Ultrasound and Ultrasound guided FNAC in prediction of lymph node metastasis in oral squamous cell carcinoma. our study came with the conclusion that the palpation, CT Scan and US are equally accurate but the US-FNAC is the most accurate technique in assessing the cervical lymphnode metastasis in oral squamous cell carcinoma.

Preface In 1938 the League of Nations Health Organization published an "Atlas illustrating the division of cancer of the uterine cervix into four stages according to the anatomo-clinical extent of the growth". Since this work appeared, the idea of visual representation of the anatomical extent of malignant tumours at the different stages of their development has been repeatedly discussed. At its meeting in Copenhagen in July 1954, the VICC adopted as part of its programme "the realization of a clinical atlas". However, the time to do the planned illustration work was not ripe until the National Committees and international organiza tions had officially recognized the 28 classifications of malignant tumours at various sites as presented in the 3rd edition of the "TNM Booklet" (1978) edited by M. Harmer. This was all the more important since in 1980, in addition to the "Booklet", a "Brochure of Checklists" edited by A.H. Sellers was made available as a further aid in the practical application of the TNMsystem
