Book excerpt: U.S. military personnel are required to adhere to standards of body composition, fitness, and appearance to achieve and maintain readinessâ€"that is, the maintenance of optimum health and performance so they are ready for deployment at any moment. In 1992, the Committee on Military Nutrition Research reviewed the existing standards and found, among other things, that the standards for body composition required for women to achieve an appearance goal seemed to conflict with those necessary to ensure the ability to perform many types of military tasks. This report addresses that conflict, and reviews and makes recommendations about current policies governing body composition and fitness, as well as postpartum return-to-duty standards, Military Recommended Dietary Allowances, and physical activity and nutritional practices of military women to determine their individual and collective impact on the health, fitness, and readiness of active-duty women.

Book excerpt: The incidence of stress fractures of the lower extremities during U.S. military basic training is significantly higher among female military recruits than among male recruits. The prevalence of this injury has a marked impact on the health of service personnel and imposes a significant financial burden on the military by delaying completion of the training of new recruits. In addition to lengthening training time, increasing program costs, and delaying military readiness, stress fractures may share their etiology with the longer-term risk of osteoporosis. As part of the Defense Women's Health Research Program, this book evaluates the impact of diet, genetic predisposition, and physical activity on bone mineral and calcium status in young servicewomen. It makes recommendations for reducing stress fractures and improving overall bone health through nutrition education and monitored physical training programs. The book also makes recommendations for future research to evaluate more fully the effects of fitness levels, physical activities, and other factors on stress fracture risk and bone health.

These data will be combined with comparable data from a Naval Health Research Center survey of active-duty Navy and Marine Corps personnel to form a comprehensive dataset for the Total Force. Principal study objectives are to (a) examine the health status of military women and men in six general areas: reproductive health, medical history and nutritional status, mental health, lifestyle factors, occupational/environmental risks and stressors, and use of health services; (b) examine the effects of military women's and men's physical health conditions or emotional problems on military work; (c) examine the impact of military service on the health status of military women and men; and (d) examine factors associated with health care utilization, satisfaction, and access to health services. This Annual Report discusses major activities of Year 1, including questionnaire development and pilot testing, sample design, and data collection planning. In addition, it notes research activities planned for Year 2.

Book excerpt: The activities of the Food and Nutrition Board's Committee on Military Nutrition Research (CMNR, the committee) have been supported since 1994 by grant DAMD17-94-J-4046 from the U.S. Army Medical Research and Materiel Command (USAMRMC). This report fulfills the final reporting requirement of the grant, and presents a summary of activities for the grant period from December 1, 1994 through May 31, 1999. During this grant period, the CMNR has met from three to six times each year in response to issues that are brought to the committee through the Military Nutrition and Biochemistry Division of the U.S. Army Research Institute of Environmental Medicine at Natick, Massachusetts, and the Military Operational Medicine Program of USAMRMC at Fort Detrick, Maryland. The CMNR has submitted five workshop reports (plus two preliminary reports), including one that is a joint project with the Subcommittee on Body Composition, Nutrition, and Health of Military Women; three letter reports, and one brief report, all with recommendations, to the Commander, U.S. Army Medical Research and Materiel Command, since September 1995 and has a brief report currently in preparation. These reports are summarized in the following activity report with synopses of additional topics for which reports were deferred pending completion of military research in progress. This activity report includes as appendixes the conclusions and recommendations from the nine reports and has been prepared in a fashion to allow rapid access to committee recommendations on the topics covered over the time period.

Data are drawn from the six Worldwide Surveys of Substance Abuse and Health Behaviors Among Military Personnel. Year 3 analyses examined interrelationships among stress, substance use, and coping, and compared military women and men and subgroups of military women. Analyses resulted in three papers and four presentations at a professional meeting. Each addressed an aspect of health and military women consistent with the grant's objectives. Specifically, among military women and men, papers examined the relationships between stress and substance abuse; the effects of stress, symptoms of depression, and coping style on occupational impairment, and relationships between deployment and substance abuse. Presentations examined deployment and substance use among military women and men; substance use and injury among military women and men; sources and effects of stress on work performance of military women and men; and the co-occurrence of substance use and other health-risk behaviors among military women and men. Copies of each are included in the appendices to the report.

Combat readiness in military women creates a unique set of health care requirements; For adult females, feminine hygiene practices constitute health care practices based on physiological necessities for the management of elimination products, including urine, feces, and menstrual discharge. This study is designed to investigate and to make recommendations for female health practices carried out in combat and non-combat environments by military women. In order to determine the best procedure(s) for maintaining feminine hygiene in combat environments, it is necessary to explore past and current practices and to obtain the recommendations of health care professionals. Both quantitative and qualitative research methodologies will be used to explore feminine hygiene practices. Phase I, the qualitative section, has been completed and being used to format Phase II, the questionnaire.

Book excerpt: The psychosocial health and well being of military women remains a critical readiness issue and there are many aspects of military duties, military operational environments, and military service across individual and family lifestages that warrant continued study. The DWHRP grant to IASWR resulted in important initial accomplishments, including the identification of specific areas of needed research and possible mechanisms for military civilian academic research partnerships. Three interrelated issues underlay the rationale and need for the activities carried out under this grant. First, there are increasing numbers of women serving in the Armed Services in expanding occupational roles. Women's service in these roles raises numerous physical and psychosocial health related concerns. Second, social workers are major providers of a broad range of health and support services for military women and their family members. Thus, social workers are in a strategic position to identity critical research questions and to conduct research that will enhance the effectiveness of health and quality of life services for military women (as well as men). Third, there is a critical need to expand research capacity in the social work profession, including a need to increase opportunities for collaboration between practitioners and researchers conducting applied research.

Book excerpt: Women Veterans are among the fastest growing groups of new VA health care users of the VA healthcare system, and currently reflect approximately eight percent of all U.S. Veterans. With Operation Enduring Freedom and Iraqi Freedom (OEF/OIF), women comprise a larger percentage of the military (11.3 percent) than of prior military operations. As of fiscal year 2010, 51.3 percent of female OEF/OIF Veterans had enrolled in VA health care, in sharp contrast to women from previous eras (an estimated 11 percent). Of this group, 88 percent have used VA health care more than once. To better understand the needs of this rapidly growing group, women Veterans' health research has expanded as well. Two previous systematic reviews examined the literature on women Veterans' health and health care up to 2008. In this review, we specifically explored women Veterans' post-deployment health, with two main areas of concentration: 1) post-deployment effects on reproductive health for women Veterans, and 2) post-trauma sequelae among women Veterans from the OEF/OIF cohort. The published literature on deployment and post-deployment health was included in the two previous systematic literature reviews. Most of the extant literature focused on the population of Veterans from OEF/OIF. The previous review also examined the reproductive health literature among military women and women Veterans. However, the previous reviews assessed the literature up through 2008, while the current review further updates these topics by examining them in more detail and extending the literature search through 2010.
