Book Supporting Clinical Practice Guidelines Development

Download or read book Supporting Clinical Practice Guidelines Development written by I. Savoie and published by B.C. Office of Health Technology Assessment, Centre for Health Services & Policy Research, University of British Columbia 1997.. This book was released on 1997 with total page 90 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report presents the results of the appraisal of selected cholesterol testing guidelines provided to the BC Council on Clinical Practice Guidelines in 1995. The aim of the appraisal was to review and evaluate the process used in developing selected cholesterol testing guidelines and to determine the extent to which these existing guidelines were "evidence-based". For the purpose of this report, cholesterol testing refers to the broader range of lipid tests that are considered in the current testing guidelines. In order to first identify existing cholesterol testing guidelines, a systematic search of the published and unpublished literature was conducted. Seventeen guidelines from various associations, governments and research bodies were uncovered. The discrepancy among the recommendations formulated by these groups was impressive. The recommendations differed in terms of the populations to be tested, the tests to be used, and the frequency of testing. For example, the US National Cholesterol Education Programs (NCEP) recommended that all adults aged 20 to 65 have their total cholesterol and HDL-cholesterol tested every two to five years. On the other hand, the Effective Health Care Research Team (EHCRT) in the UK concluded that cholesterol screening of the general population should be actively discouraged, as it will not make a contribution to lowering overall mortality. To understand the origins of these wide discrepancies and to evaluate the extent to which existing cholesterol testing CPGs were evidence-based, BCOHTA used a set of 15 appraisal criteria, pertaining to clinical practice guidelines, derived from work done by the Institute of Medicine and Agency for Health Care Policy and Research. BCOHTA concluded that the EHCRT guidelines were the only guidelines in which the recommendations were internally congruent with the research evidence. Problems in linking the research evidence to the recommendations appeared to be a major weakness in the developme

Book excerpt: This timely, concise title provides an important update on clinical lipid management. Using information from recent clinical trials and in special populations, the book begins by offering an easy-to-read overview of LDL, HDL, and triglyceride metabolism and the genetics of lipid disorders. The link between inflammation and lipids, and how this relates to atherosclerosis development, is also addressed, as are the measures of subclinical atherosclerosis in patients with abnormal lipid levels. Lipid abnormalities in children, with a particular focus on vulnerable populations (with an emphasis on ethnicity and childhood obesity), are covered. The treatment goals and approaches for managing lipids in the clinic are thoroughly discussed, emphasizing the important role of statin use and addressing controversies of lipid management in special populations such as heart failure, end stage kidney disease and fatty liver disease. Of special note, an important update on how new HIV medications impact lipid levels is provided. In all, Lipid Management: From Basics to Clinic, is an invaluable, handy resource for understanding changes in lipids in different populations and for sharpening the clinical approach to managing complicated lipid cases.
