L'Organisation Mondiale de la Santé encourage le développement de la rééducation et réadaptation palliative (RRP), une prise en charge multidisciplinaire visant à améliorer la qualité de vie des patients en optimisant leurs capacités fonctionnelles et leur bien-être psychosocial. Cependant, en situation palliative la notion de temporalité est primordiale. Il est essentiel de réfléchir aux bénéfices potentiels pour chaque patient avant de proposer une RRP, surtout en cas d'hospitalisation complète, tout en veillant à ne pas empiéter sur le temps qu'ils peuvent passer auprès de leurs proches. Disposer d'indicateurs fiables pour estimer les bénéfices fonctionnels possibles par la RRP serait une plus-value pour élaborer un projet de soins adapté au projet de vie du patient et à son pronostic. Cette étude vise à analyser les facteurs pouvant être prédictifs d'une amélioration fonctionnelle après une RRP. MÉTHODE : Il s'agit d'une étude rétrospective monocentrique portant sur les patients atteints d'un cancer avancé hospitalisés dans le service de SMR Soins de Supports du CHU de Rouen. L'amélioration fonctionnelle a été définie par une augmentation d'au moins 10% du Palliative Performance Scale à l'issue du séjour. Les données anthropométriques, cliniques, médicales et biologiques à l'entrée dans le service ont été analysées. RÉSULTATS : Parmi les 40 patients inclus, 16 ont eu une amélioration fonctionnelle. L'analyse de régression logistique univariée a mis en évidence deux variables significatives : la CRP (OR = 0_97 ; IC95% [0_95 ; 0_99] ; p = 0.243) et le score pronostic palliatif de survie Pronopall (OR = 0.69 ; 1C95% [0.49: 0.98] ; p = 0.397]. Dans l'échantillon d'étude, on a constaté que 83.3% des patients naïfs de tout traitement ontologique et 66.7% des patients n'ayant eu aucune hospitalisation non programmée dans les 12 derniers mois présentaient une amélioration fonctionnelle. CONCLUSION : La CRP et le score Pronopall sont des variables importantes à explorer et qui pourraient aider à l'orientation des patients atteints d'un cancer avancé vers des programmes de rééducation-réadaptation fonctionnelle. L'intérêt d'une préhabilitation avant chimiothérapie palliative pourrait être une piste à développer pour les patients diagnostiqués d'emblée avec un cancer incurable. Avant tout, une collaboration précoce entre les oncologues et les équipes pluridisciplinaires de soins palliatifs est à promouvoir.
