The book addresses three broad questions: How is children's health care currently financed? Does insurance equal access to care? How should the nation address the health needs of this vulnerable population? America's Children explores the changing role of Medicaid under managed care; state-initiated and private sector children's insurance programs; specific effects of insurance status on the care children receive; and the impact of chronic medical conditions and special health care needs. It also examines the status of "safety net" health providers, including community health centers, children's hospitals, school-based health centers, and others and reviews the changing patterns of coverage and tax policy options to increase coverage of private-sector, employer-based health insurance. In response to growing public concerns about uninsured children, last year Congress voted to provide $24 billion over five years for new state insurance initiatives. This volume will serve as a primer for concerned federal policymakers and regulators, state agency officials, health plan decisionmakers, health care providers, children's health advocates, and researchers.

Book excerpt: Introduction: Over the past five years, California made significant strides in reducing the number of uninsured children from an estimated 778,000 in 2003 to 683,000 children in 2007, a 12 percent decline. This improvement in children's coverage was due in part to Medi-Cal and Healthy Families expansions, the introduction of Healthy Kids programs in several counties, and effective outreach and enrollment efforts. However, gains in children's coverage have slowed in recent years. Children's Health Coverage Facts and Figures provides an overview of trends in children's coverage and insurance programs in the state. Key findings include: The proportion * of children without health insurance continued to decline through 2007, though the pace of improvement has slowed. * Nearly 80 percent of California's uninsured children are eligible for coverage under either Medi-Cal, Healthy Families, or Healthy Kids. * Medi-Cal and Healthy Families are key sources of coverage for children in low-income households that together have closed the coverage gap among families with incomes up to 250 percent of the federal poverty level. * Healthy Kids programs are also important for children's coverage. Twenty-four counties operate Healthy Kids programs and four others rely on California Kids. * Children are less likely to have employment-based coverage than adults and are more likely to be enrolled in public programs in California. The future of children's health insurance coverage in California is uncertain. On the one hand, California's economic downturn and budget crisis will likely make it difficult to increase the number of insured children. On the other, Congress and the President reauthorized the Children's Health Insurance Program (CHIP), which provides new policy options for expanding coverage. In addition, Congress and the Obama Administration are working on national health care reform, which if enacted, would likely benefit children in California. In this presentation, children are defined as people between the ages of 0 and 18, unless otherwise noted.

Book excerpt: Losses in employer-provided health insurance among kids have serious consequences for the children themselves, families, communities, businesses, and the public insurance system. Th is Briefing Paper documents the ways in which children without employment-based coverage affect those around them, and it provides further evidence for the need for important legislative changes in California and the United States as a whole. The first section outlines the state of employer-provided health insurance for kids in California and illustrates recent trends in coverage across various sub-populations. Such an analysis highlights the strengths, and more importantly the weaknesses of the current system, and points to the populations who have traditionally been left out and thus need to be targeted in future policy decisions. This paper then examines the effects fewer insured children have on the community, businesses, and the public insurance system. It is important to understand the widespread effects uninsurance among children has on those besides themselves and their families. Their delay in treatment, lack of disease management, and use of emergency rooms ultimately are detrimental to community and schools, working families and through them business productivity, and the budgets of the public insurance system. If fully funded and implemented effectively, these public systems can provide as good as, and in some cases even more efficient than the employment-based system. This report concludes by addressing specific policy prescriptions that can help ameliorate these problems.

In the United States, children with access to healthcare are more likely to have better health outcomes, live healthier lives, and do better in school. Although many children now have access to healthcare insurance, this has not always lead to these children having access to healthcare. Although state and federal healthcare reforms are addressing issues of providing children access to health insurance, many children will continue facing severe issues with acquiring access to healthcare, especially those enrolled in Medi-Cal. In this thesis, I analyze possible solutions for addressing healthcare access needs for California's children enrolled in Medi-Cal. I first review articles and published studies to develop a list of possible factors that are correlated with children being more likely to lack access to healthcare that include specific: Healthcare insurance factors; child demographic factors; and family/parental factors. Further, I review articles and published studies to develop a list of possible alternatives to addressing children's healthcare access needs including specific: Healthcare infrastructure alternatives, healthcare delivery model transformation alternatives, and finance alternatives. To analyze my research question: "what policies should California implement to address the healthcare access needs of the state0́9s child population in the Medi-Cal system?", I employ a criteria alternatives matrix (CAM) analysis model to examine which of the potential alternatives can address the factors that are correlated with children being more likely to lack access to healthcare. I further use the knowledge from the review of articles, published studies, and independent analysis to develop specific recommendations on policies the state can develop to implement the alternatives. Based on my analysis I found that there are various actions policymakers can take to ensure children enrolled in Medi-Cal throughout the state receive adequate access to healthcare. Two alternatives: expanding the use of community-based collaboratives, and expanding the use of school-based health centers are highly recommended as they would address the most barriers to children's healthcare access. The other eight alternatives each address different barriers to children's healthcare access that could benefit children in specific regions and/or populations if implemented. Ultimately policymakers should look at all options outlined and implement what is most appropriate at the state and local levels, considering that inaction is not an acceptable option as the health of our children depends on it.

It concludes with the finding that uninsured children have worse access to and use fewer health care services than children with insurance, including important preventive services that can have beneficial long-term effects.
