Cancer of the pancreas is abnormal cell growth in the tissue of the pancreas which is a small organ (about six inches long) located next to the small intestine, behind the stomach. The pancreas has two main jobs in the body: to produce juices that help digest (break down) food and to produce hormones, such as insulin and glucagon, that help control blood sugar levels. Both of these hormones help the body use and store the energy it gets from food. The majority of pancreatic cancers are adenocarcinomas. Adenocarcinomas are tumours that arise from the exocrine portion of the pancreas. The exocrine portion of the pancrease produces digestive fluids. A small number of pancreatic cancers begin in the endocrine pancreas. The endocrine portion produces hormones (such as insulin). Despite the high mortality rate associated with pancreatic cancer, its aetiology is poorly understood, it is rarely curable and has an overall survival rate of less than 4%. Research on changing the survival rates of this stubborn disease is highlighted in this new book.

In 2007, a Clinical Trials Planning Meeting on Pancreas Cancer Treatment was convened to discuss the integration of basic and clinical knowledge in design of clinical trials in pancreatic adenocarcinoma. A Consensus Report of the meeting published in November 2009 provided recommendations in the following emphasis areas: New Targets for Drug Development; Utility of Preclinical Models; Future Clinical Trials; Establishment of Biorepositories; Development of Biomarkers. NCI's efforts in pancreatic cancer research have increased our understanding of the biological basis for pancreatic cancer, developed potential means to reduce morbidity and mortality associated with the disease, and expanded pancreatic cancer research capacity. The following or some research highlights in the five key priority areas identified by the PRG: Tumor Biology – Research on the tumor biology of pancreatic cancer has increased understanding of factors critical to key steps in the progression of pancreatic cancers; Risk, Prevention, Screening, and Diagnosis – Results of NCI-sponsored research related to the risk, prevention, screening, and diagnosis of pancreatic cancer have identified potential markers associated with pancreatic cancer risk as well as potential diagnostic markers; Therapy – NCI-sponsored research on therapy for pancreatic cancer has investigated optimal timing, delivery, and drug combinations that may improve the treatment of pancreatic cancer.~

Every year, about 33,700 people in the United States will be diagnosed with pancreatic cancer and over 32,000 patients will die from the disease. The median survival of patients with advanced pancreatic cancer is about 6-months. This dismal picture of pancreatic cancer is mainly due to the lack of early diagnosis and effective treatment for patients with advanced disease. To increase the survival rate of pancreatic cancer patients, better tumor markers for diagnosis and new molecular targets for drug development are desperately needed. A lot of effort has been made in searching for pancreatic cancer-causing genes or genes associated with progression of malignant behavior in pancreatic cancer. As a result, alterations in the expression of several cancer-related genes have been identified in pancreatic tumors. The identification and characterization of these cancer-related genes have significantly increased our understanding of pancreatic cancer development, but unfortunately the treatment of pancreatic cancer has not advanced as much in the past 20 years. Over the past decade, tremendous advances have been made in the field of cancer drug discovery, particularly, in the area of molecular and genetic models and technologies. Many of those advanced models and technologies have been applied to the drug discovery processes for pancreatic cancer. In this book, a team of experts will describe the latest development in the application of these models and technologies in pancreatic cancer. The authors include basic researchers as well as clinicians who work in the front-line of the war against pancreatic cancer and have the first-hand experience on these cutting-edge tools and techniques. The book can be divided into two general areas: 1) model systems and 2) genomics and proteomics tools.
