But having supplied the proper kind and amount of aliment, we must place our patient in condition to digest and assimilate it. It is true, as already suggested, that we may make use in certain measure of predigested foods, and that we may assist digestion in other instances by the administration of the digestive ferments; but our endeavors must not cease there. He performed experiments and autopsies, became interested in the new chemistry of Lavoisier, risked scorn in his use of alkaline remedies, studied epidemic fever and approaches to bloodletting, and struggled to understand epidemic fever, childbed fever, cancer, public health, consumption, mental illness, and the "dangers of spirituous liquors." Dr. Barker intended to publish his Diseases in the District of Maine 1772-1820 by subscription - advance pledges to purchase the published volume - but for reasons that remain uncertain, that never happened. For the first time, Barker's never before published work has been transcribed and presented in its entirety with extensive annotations, a five-chapter introduction to contextualize the work, and a glossary to make it accessible to 21st century general readers, genealogists, students, and historians. Tubercle in its varied protean guises, is but the result of a deterioration of the blood, of the retention of excre tions, carbonaceous and other impurities in the blood, where they have no business to remain. In oonss quence of the imperfect performance of the respiratory functions, these impurities accumulate. The time at length comes round when they must be got rid of, if not during and through the act of respiration, perforce otherwise. The result is their deposition as an inorganic matter in the form of tubercle, in the lungs and other or gans, these, saving the diffusion of tuberculous blood, being perchance in other respects sound. A dead matter is de posited in the tissues. That which Should be thrust aside, Shovelled out as it were, is detained Within the organisa tion. These all-important facts being positively determined, the cause and radical cure, in Short the prevention of consumption and scrofula with all their concomitant ravages, are placed as absolutely as small-pox itself has been placed, within human control. This is technically denominated tubercle, hence the name Tubercular Consumption, signifying the same thing as Phthisis Pulmonalis, and Pulmonary Consumption, but seldom used, except when physicians talk with each other, or wish to make a flourish in the presence of their patients, with out being understood. These tubercles vary, not a little, in size, form, color, and consistence. They are found no larger than millet-seeds and small peas, up to the size of a hen's egg; roundish or irregular in form; yellowish, gr'ayish, or dull dark brown; isolated or clustered in bunches; of a hard consist ence, or so soft and friable that they easily break down be tween the thumb and finger.
