Book excerpt: Multiple sclerosis [MS] is one of the most common auto-immune-mediated diseases of the human central nervous system [CNS] which affects young adults and usually causes significant neurological disability. Currently, the causes of MS are still unclear, a cure for MS remains elusive and the effectiveness of treatment varies significantly among individuals. Clinical features and neurological deficits stemming from this progressive neurological disease are diverse since MS potentially affects human CNS at all levels from brain to the end of spinal cord. The triggering event for development of MS remains unknown. Immunopathogenesis of MS involves a number of steps which include activation of peripheral leukocytes against putative CNS antigen(s), interactions of the activated leukocytes with inflamed cerebral endothelial cells, transendothelial migration of activated lymphocytes and macrophages to the CNS milieu, and further propagation of the massive immune response within the CNS. Such massive immune activation leads to loss of myelin-oligodendrocyte complex. Several immune cell types and mediators of the immune-inflammatory response actively contribute to pathogenesis of MS. Genetic factors are also believed to play a central role in the development of most forms of MS. Another important but much unrecognized and under-researched feature of MS immunopathology is “neurodegeneration.” Neuronal loss and axonal degeneration are the core components of irreversible and permanent CNS atrophy and disability in MS. What we call MS in reality is a heterogeneous group of diseases and at least four distinct immunopathological subtypes of MS with dissimilar responses to therapy with immunomodulatory agents exist. MS is a clinical diagnosis, however, its diagnostic process is much facilitated by utilization of laboratory and neuroimaging studies. Present therapies of MS are either immunomodulatory agents or immunosuppressive and mainly target the peripheral immune system with the intention to ameliorate the severity of acute relapses, decrease annual relapse rate, and improve MRI lesions. Currently, much research activity is being conducted to better understand the fundamental disease mechanisms of MS and find more effective and safer treatments for this incurable disease. This book presents an overview of MS as a disease with neuroinflammatory and neurodegenerative features and the authors discuss the most recent findings about MS and its treatment. Great progress has been made in identifying the molecular mechanisms that can be utilized to develop highly efficacious treatments for MS patients. Since the middle of the last century, when studies using an inflammatory autoimmune model of MS i.e. experimental autoimmune encephalitis (EAE) were initiated, research on MS has since led to an enormous growth of knowledge. The limitations arising from these animal models have been partially compensated by insights generated from histopathological, immunological, genetic, imaging and clinical studies of MS. The importance of T cells and, recently of B cells, in MS has been elucidated and the significance of CNS resident cells, particularly in the progressive course of MS has been highlighted. The importance of environmental factors and genetic predisposition have also been recognized and the interaction between th e individual and its environment seems to play key roles in the pathogenesis of this disease. However, definite causative agent(s) or single gene(s) involved in MS still remain unidentified. A deeper understanding of the immunological pathways involved goes hand in hand with further improving therapy for MS. Focal inflammation in MS, caused by de novo CNS infiltration, can be prevented effectively using peripherally-acting drugs. However, diffuse CNS intrinsic inflammation and neurodegeneration are not targeted by the current arsenal of therapeutics and patients with progressive disease courses remain difficult to treat. Generating insights into pathophysiological processes and the clinical translation of this knowledge have been a formula of success for treating inflammatory diseases. The future will show whether this will hold true for CNS-trapped inflammation and neurodegeneration and whether the ultimate goal - truly curing MS - will be possible.

Fatigue is perhaps the single most prevalent and disabling symptom of the multiple sclerosis, and limits patients' activity more than any other MS symptom. The identification of fatigue as a distinct clinical entity requires both art and science, and most of all, a willingness and ability to listen carefully to patients and their families. The physician's ability to obtain a comprehensive history requires a full understanding of the circumstances in which fatigue occurs (physical, cognitive, and psychosocial) and demands consideration of a large number of disorders, including anxiety, depression, excessive daytime sleepiness, pain, and spasticity, all of which may mimic or contribute to fatigue. While fatigue is almost exclusively a subjective experience, depending on the patient's ability to understand and report this symptom, there are effective methods for identifying the existence of fatigue, determining its severity, and distinguishing it from related or contributing disorders such as depression, pain, and sleep disorders. Readers will learn that fatigue need not be tolerated by the MS patient. Various therapies, support systems, and treatment of underlying affective disorders can all alleviate fatigue or reduce its impact, restoring the patient's energy levels and ability to participate in life. Virtually every MS patient with fatigue can benefit from intervention, and failing to treat the symptom of fatigue with the respect that it deserves is a serious detriment to patient care.
