It is at the heart of the inclusive governance needed for countries to stake their individual paths towards Universal Health Coverage while ensuring that no one is left behind. As simple as it may seem in theory, it is a complex undertaking in practice, one which policy-makers struggle with. The Handbook on Social Participation for UHC is thus designed to provide practical guidance, anchored in conceptual clarifications, on strengthening meaningful government engagement with the population, communities, and civil society for national health policy-making. It draws on best practices and lessons learned to support government institutions in setting up, fine-tuning, improving, and institutionalizing new or existing participatory health governance mechanisms. The handbook follows through the different tasks which policy-makers must reflect on and undertake when bringing in people’s voice into health policy-making. Examples include creating an enabling environment for participation, ensuring good representation, strengthening capacities, increasing policy-uptake of participatory process results, and sustaining participatory engagement over time. A fundamental premise of the handbook rests on the idea that policy-makers can leverage format and design elements of a participatory process to address power dynamics amongst participants, thereby fostering more meaningful contributions to the process.

As such examples suggest, the activities of many governmental groups and professional advisory bodies, although varied, tend to converge upon a number of especially important issues. If one peruses the index of documents discussed in Volume Four, certain topics are more often the focus of legislation and official concern than others: withholding and withdrawing treatment, access to health care, consent to treatment and experimentation, and issues posed by HIV testing and AIDS. Such a common focus should not be exaggerated, for the discussion of topics is wide-ranging. But that commonality, when in evidence, is also not surprising. It suggests that key issues and concerns in bioethics may be widely shared among modern cultures and societies, for all the distinctiveness of a particular nation's or region's response to them. Issues of informed consent, after all, implicate more fundamental matters of respect for persons and the rights of individuals in the contexts of therapy and research. Issues of access to medical care concretize deeper questions about the nature and scope of a society's welfare obligations to its citizens.

The Political Economy of Same-Sex Marriage: A Feminist Critique considers this debate from a political economy perspective. Rather than engaging directly in the now well-rehearsed social-movement and academic for-and-against debates, this book focuses on processes of institutionalization of same-sex marriage and so-called "rainbow families" within (neo)liberal capitalist democracies. It examines how states and markets appropriate same-sex marriage and family to enhance their own political and symbolic capital, consolidating power and profit within existing systems of gendered and raced socioeconomic stratification. Taking a radical feminist, heterodox, qualitative and intersectional approach, this book investigates the political economy of same-sex marriage across three axes: same-sex marriage as institution; same-sex marriage and the market; and the political economy of the "rainbow family". The examination of case studies from different countries and regions enables a comparative analysis that foregrounds cultural, political and economic path dependencies while at the same time highlighting a number of striking commonalities. In all the countries discussed in this book and in most respects, same-sex marriage has been integrated almost seamlessly into a mainstream/malestream political economy of marriage and family and its translation into added market and productive value. The Political Economy of Same-Sex Marriage: A Feminist Critique will be of use to researchers and students alike, and indeed to all those who are curious about the mainstreaming of homosexuality within twenty-first-century capitalist democracies.

Book excerpt: Mitochondrial replacement techniques (MRTs) are designed to prevent the transmission of mitochondrial DNA (mtDNA) diseases from mother to child. While MRTs, if effective, could satisfy a desire of women seeking to have a genetically related child without the risk of passing on mtDNA disease, the technique raises significant ethical and social issues. It would create offspring who have genetic material from two women, something never sanctioned in humans, and would create mitochondrial changes that could be heritable (in female offspring), and therefore passed on in perpetuity. The manipulation would be performed on eggs or embryos, would affect every cell of the resulting individual, and once carried out this genetic manipulation is not reversible. Mitochondrial Replacement Techniques considers the implications of manipulating mitochondrial content both in children born to women as a result of participating in these studies and in descendants of any female offspring. This study examines the ethical and social issues related to MRTs, outlines principles that would provide a framework and foundation for oversight of MRTs, and develops recommendations to inform the Food and Drug Administration's consideration of investigational new drug applications.
