Book excerpt: Excerpt from Radium Therapy in Cancer at the Memorial Hospital, New York: First Report: 1915-1916 The somewhat detailed character of the present report has been determined by the urgent need of more accurate descriptions of the methods of applying radium to malignant tumors in different situations. The great majority of available reports indicate the total amount of radium employed, but details regarding concentration of radium to the unit of surface, the exact nature of filtration, the exact periods of repeated applications, and the successive steps of the therapeutic result, are incompletely presented or entirely omitted. Consequently it has been impossible for the reader to duplicate the method or to obtain the necessary information to guide him in his own work. Unfavorable results are experienced in all large radium clinics, but under exactly what circumstances such results may be expected, or how they may be avoided, has seldom been stated. Such information, it is believed, may be obtained from many of the case records of the present report. It has become more and more apparent that the successful use of radium in cancer, requires careful consideration of each particular type of the disease in each organ as separate problems, in which different methods must be devised and different results expected. Hence the various forms of cancer are separately considered, and the indications, methods, complications, and results are given briefly for each. In our experience the use of radium emanation has greatly increased the efficiency of radium therapy as compared to the use of the metal itself. For a satisfactory understanding of these advantages as well as the principles of application and filtration an acquaintance with the physics of radium is indispensable. It has therefore been deemed advisable to include in this report an introduction explaining in detail the physical considerations relative to the therapeutic application of radium. The author desires to express his indebtedness to Dr. Samuel Brown, to whose efficient services in making and changing the applicators and in following the patients much of the success of this work is due. The reception accorded to the writer's first attempt at authorship has been a matter of considerable gratification. The valuable suggestions of kindly critics have led to the addition of much new matter in the present volumes, and have encouraged him to venture further afield in the most interesting of the new additions to the science of medicine. The sections dealing with the military side of radiography and radio-therapeutics have been consider ably enlarged. The revision oi the text under war conditions has not been easy owing to the urgent calls of work in connection with military duties, and the author claims the indulgence of his readers for any shortcomings in the present edition due to this cause. On the other hand, the unique opportunities offered by the vast amount of work dealt with have enabled him to add much new matter which may be helpful to other workers at the present time. The illustrations have been critically gone through and revised, and this has resulted in the addition of 24 plates and 150 illustrations in the text. Wherever possible acknowledgment has been made in the text of the sources from which new matter has been obtained, but I should like here to record my indebtedness to the work of A. E. Barclay on the Oesophagus and Stomach. By nasopharyngeal fibroma is meant that rare form of growth which, composed mainly of connective tissue and highly vascular, usually takes its origin from the basilar process of the occipital bone, including the natural passages, creating pressure upon the parts in its vicinity, and in many cases giving rise to more or less frequent and abundant hemorrhages. The period of the greatest activity of its development is about the time of adolescence. Following this, the tendency to its enlargement diminishes and, in fortunate cases, if enough time be allowed and the conditions due to the presence of the tumor be not too urgent, it may gradually disappear. Such a result, however, is unusual. In nearly every instance the conditions arising from the presence of the growth are so serious as urgently to demand relief. This has been recognized for many generations. And a great' number of methods of treatment for it have been devised. Book excerpt: Excerpt from Radium, Vol. 4: February, 1915 Radium is likely to play its role so much the better, the more the neoplasm has been reduced to an appreciable minimum by a surgical operation. We may say also that the latter may be facilitated or even rendered possible by radium, which, in cer tain cases, modifies an inoperable neoplasm to such an extent that it becomes operable. Such cases, above all others, would be amenable to post-operative irradiation. In spite of our long period of personal practice we have been unable to formulate any statistics, owing to the fact that the cases differ so much from one another, and we can here' give only a general summary of facts reflecting the general impression which has been made upon us by the numerous cases of cancer of the uterus or of recurrence which we have treated by radium. Excluding cases in which the lesions have deeply invaded the true pelvis and the ganglia, and those in which extension to the bladder and rectum render any attempt at complete treatment of the neoplasm impossible, it may be said that the distressing symptoms and haemorrhage have invariably been greatly relieved by the application of radium. Although we emphasize the value of radium therapy, we do not for a moment intend to claim that radium plays the role in the cure of cancer. Our present object is to render obvious its value as a new therapeutic agent, which is capable of rendering services to the patient, such as can be rendered by no other method of treatment, excepting always those which can be ren dered by surgery in operable cases. Without wishing to revert here to the special practical facili ties oi the armamentarium of radium therapy, we think it, desir able, whilst doing justice to the painlessness of the applications, to point out a few manifestations inherent in the treatment, which it is essential to recognize and take into consideration. Even during the applications the patients may experience a sen sation of nausea, which in certain cases results in vomiting, but which otherwise disappears very rapidly.
