Book excerpt: L'objectif de cette étude est de quantifier des indices construits à partir de documents radiographiques panoramiques, selon des modalités validées par la littérature internationale, chez des consultants d'un service hospitalier d'urgences odontologiques, et de comparer les résultats obtenus à ceux de travaux faisant actuellement office de référence pour représenter la population générale. Les données radiographiques sont recueillies sur une population de 177 patients adultes venus consulter en urgence au sein de l'unité fonctionnelle d'odontologie du groupe hospitalier Pitié Salpetrière de Paris (Assistance Publique - Hôpitaux de Paris), rattaché au service d'Odontologie de Garancière de l'Hôtel Dieu de Paris, durant la semaine du lundi 02 Février 2004 au vendredi 06 Février 2004. Sont étudiés l'âge et le genre des consultants, le diagnostic de l'urgence, l'analyse radiologique par lecture du bilan panoramique radiographique et la fréquentation du service d'urgence. Les résultats de cette étude montrent que la grande majorité des consultations d'urgences analysées, qui concernent un public jeune, à prédominance masculine, sont médicalement justifiées par l'existence de douleurs ou d'épisodes infectieux aigus (83%). Les deux tiers d'entre elles auraient cependant pu être évitées par une prise en charge thérapeutique appropriée plus précoce. La santé bucco-dentaire des consultants est significativement moins bonne que celle de la population générale, en référence aux travaux permettant une comparaison pertinente. L'analyse de cette santé bucco-dentaire souligne d'un strict point de vue clinique l'influence du renoncement aux soins sur la fréquentation des consultations d'urgence hospitalières, suggérant l'importance des mesures de Santé Publique favorisant l'accès aux structures de soins de droit commun.
