Book excerpt: This study considers the radiation safety aspects of several designs discussed in a previous report of an accelerator-based source of neutrons, based on the [sup 7]Li(p, n) reaction, for a Boron Neutron Capture Therapy (BNCT) Facility at Lawrence Berkeley National Laboratory (LBNL). determines the optimal radiation shield thicknesses for the patient treatment room. Since this is an experimental facility no moderator or reflector is considered in the bulk wall shield design. This will allow the flexibility of using any postulated moderator/reflector design and assumes sufficient shielding even in the absence of a moderator/reflector. In addition the accelerator is assumed to be capable of producing 100 mA of 2.5 MeV proton beam current. The addition of 1% and 2% [sup 10]B (by weight) to the concrete is also investigated. The second part of this paper determines the radiation dose to the major organs of a patient during a treatment. Simulations use the MIRD 5 anthropomorphic phantom to calculate organ doses from a 20 mA proton beam assuming various envisioned moderator/reflector in place. Doses are tabulated by component and for a given uniform [sup 10]B loading in all organs. These are presented in for a BeO moderator and for an Al/AlF[sub 3] moderator. Dose estimates for different [sup 10]B loadings may be scaled.

Book excerpt: Boron Neutron Capture Therapy (BNCT) in the United States has entered into a new phase with the initiation of clinical trials using neutron sources at the Brookhaven National Laboratory and the Massachusetts Institute of Technology. If these trials are successful at demonstrating the efficacy of BNCT as a viable treatment for glioblastoma multiforme, then there will be an immediate demand for several additional neutron sources in order to treat the several thousand patients currently diagnosed with glioblastomas in the U.S. each year. However, the requirements for an acceptable neutron source for BNCT are rather severe in terms of the need to provide a sufficient number of epithermal neutrons to a patient-accessible location in a reasonable time with minimal thermal-neutron, fast- neutron, and gamma-ray background. A recent study of potential neutron sources at Oak Ridge National Laboratory (ORNL) has been completed, which concludes that the Tower Shielding Facility (TSF), also appears very well suited for BNCT. The light-water-cooled reactor is contained in an aluminum pressure vessel and located in a large concrete 'bunker' referred to as the Big Beam Shield (BBS). The BBS contains a 77-cm-diameter beam collimator, which permits access to a broad beam neutron flux exceeding 4 x 10[sup ll] Cm[sup -2]s[sup- 1] at the operational power of 1 MW. The collimated beam emerges horizontally onto an unenclosed test pad area on which shield mockups were assembled. The appropriate beam filter and collimator system can be easily constructed in the expansive area previously used for the large shield mockups. Additional engineering of the beam shutter mechanism and the construction of treatment support facilities will be needed but can be easily accommodated on the remote dedicated site. The filter design analysis is provided.

Book excerpt: Purpose; To describe the design and construction of an accelerator-based boron neutron capture therapy (AB-BNCT) facility with multiple treatment rooms at the Southern Tohoku BNCT Research Center (STBRC).Materials and Methods; AB-BNCT system at the STBRC is equipped with a cyclotron-based epithermal neutron source (C-BENS), which consists of a cyclotron accelerator (HM-30), a beryllium neutron production target, and a beam shaping assembly (BSA). We developed a remote patient transport system (RPTS) for workers to reduce the work time in the treatment room under the condition of remaining activities just after an irradiation. We studied the feasibility of this system and carefully designed optimum layout to realize patient flow and workflow efficiently.Results; We designed the upside-down Y shaped beamline configuration, in which HM-30 and two treatment rooms are assumed to be located on a top and bottoms, respectively. To reduce the activities caused by thermal neutron, BSA is surrounded by LiF-loaded polyethylene blocks and low-activation concrete. The measured out-of-field thermal and fast neutron dose profiles were in good agreement with calculated ones using MCNPX. It was also confirmed that the RPTS could be operated up to 9 m apart from the RPTS without any problems. Conclusion; We successfully established the environment of BNCT as one of the division of general hospital without sense of incongruity in comparison of environment of conventional radiotherapy. The AB-BNCT system described in this study confirmed to specifications and is being used for BNCT in a hospital.

Neutron capture therapy (NCT) is the binary system that has been the subject of the Fifth International Symposium on Neutron Capture Therapy, which was held September13-17, 1992, in Columbus, Ohio, undertheauspicesoftheInternational Society for Neutron Capture Therapy. Its objective was to bring together researchers from throughout the world and to provide a forum at which they could present the latest advances in the development of Neutron capture therapy. Neutron capture therapy has largely, but not exclusively, focused on the use of boron-10 as the target nuclide. Boron neutron capture therapy (BNCT) is based on the nuclear reaction that occurs when the stable isotope, boron-10, absorbs low-energy non ionizing thermal neutrons to yield alphaparticles and recoiling lithium-7 nuclei. The size and energy of these high linear energy transfer (LET) particles result in their being confined largely to the cells in which the capture reaction occurs. For BNCT to be successful, a sufficient numberof I~atoms mustbe localized within neoplastic cells, and enough thermal neutrons must be delivered and absorbed by the I~ to produce a lethal 1~(n,QVLi reaction. Two major problems must be surmounted.
