Book excerpt: INTRODUCTION : Les fractures de l'extrémité supérieure de l'humérus occupent la 3ème place des fractures ostéoporotiques des membres. La prise en charge des fractures complexes déplacées est soumise à controverse et a multitude de traitements proposés montre l'absence de prise en charge codifiée. Le but de ce travail est d'étudier les résultats fonctionnels et radiologiques des prothèses inversées d'épaule dans le traitement des fractures déplacées 4 fragments de l'extrémité supérieure de l'humérus chez la personne âgée. MATERIELS ET METHODES : Une étude rétrospective monocentrique a été réalisée de 2007 à 2011 incluant les fractures complexes (4 fragments) de l'humérus proximal déplacées chez les patients âgés de plus de 75 ans et/ou présentant une rupture massive de coiffe. Trente patients ayant bénéficié d'une anthroplastie de l'épaule avec mise en place d'une prothèse inversée Delta Xtend ont été inclus. L'âge moyen des patients était de 79,2 ans ± 9,8. Dans 90% des cas (n=27), il s'agissait de fractures 4 fragments déplacées et dans 10% de fractures-luxations (n=3). Un abord supéro-latéral a été réalisé dans 79% des cas et delto-pectoral dans 21%, 4 patients ont bénéficié d'une réinsertion des tubérosités. Les patients ont été évalués par le score de Constant, par le score de DASH et radiologiquement. RESULTATS : 28 patients ont été analysés avec un recul moyen de 22,3 mois. Le score de Constant moyen brut du côté opéré était de 43,5 points ± 17,9 (contre 76,2 points ± 10,1 du côté non-opéré), l'élévation antérieure active était en moyenne de 92,1° ± 27,3, l'abduction moyenne était de 78,6° ± 26,5. La rotation externe moyenne (cotée sur 10 points) était de 3,1 alors que la rotation interne moyenne était de 3,3. Book excerpt: Les fractures de l'extrémité supérieure de l'humérus constituent les 3èmes fractures les plus fréquentes chez les patients âgés. La prothèse inversée d'épaule est, à l'heure actuelle, le traitement de première intention des fractures complexes, comminutives, déplacées, chez ces patients, devançant l'ostéosynthèse et l'hémiarthroplastie montrant des résultats décevants. L'objectif de cette étude était de déterminer le taux de survie de ces prothèses et de le comparer aux résultats de la littérature. Les objectifs secondaires étaient de comparer les résultats fonctionnels et radiologiques chez nos patients avec la littérature. Il s'agit d'une étude de cohorte rétrospective monocentrique réalisée au sein du CHU de Limoges. Nous avons inclus 189 patients de 81 ans d'âge moyen ayant bénéficié de la mise en place d'une prothèse inversée d'épaule dans le cadre de la prise en charge de fractures déplacées de l'humérus proximal, Neer III ou IV, entre Janvier 2012 et Décembre 2021. 40 patients ont pu être revus pour une évaluation fonctionnelle et radiologique au dernier recul. Nous retrouvons un taux de survie de prothèse de 98,8% à 4,1 ans de recul moyen, avec 1 seule reprise de prothèse sur l'ensemble de la série, comparable aux taux de survie rapportés dans la littérature. Concernant les résultats fonctionnels chez les patients revus, on retrouve un score de Constant moyen de 59,4, un QuickDASH moyen de 25,4 et un SSV moyen de 70% à 36,7 mois de recul moyen. Les patients présentent des mobilités moyennes tout à fait satisfaisantes. Concernant les résultats radiologiques, on recense 77% de consolidation des tubérosités et 20 % de notching. Nous avons relevé 11,1% de complications dans notre série. Les prothèses inversées d'épaule pour prise en charge des fractures complexes de l'extrémité supérieure de l'humérus chez les patients âgés constituent donc un traitement de référence avec un taux de survie de prothèse très satisfaisant à long terme. Elles permettent un retour rapide à l'autonomie, une diminution notable de l'antalgie et une récupération fonctionnelle satisfaisante s'améliorant avec le temps. Les complications sont peu fréquentes et n'entraînent, pour la plupart, pas de conséquences fonctionnelles à moyen terme.
