La tentative de suicide de l'adolescent révèle souvent un malaise individuel et familial. Par l'identification précoce des jeunes à risque, on peut espérer améliorer la prévention du geste suicidaire. Le rôle du médecin généraliste reste essentiel dans la prise en charge et l'accompagnement du jeune et de sa famille. Un travail pluridisciplinaire en réseau, avec des professionnels formés, permet un meilleur encadrement du jeune.

Les conduites suicidaires renvoient, généralement, à un trouble grave du développement à l'adolescence, lié aux perturbations actuelles et passées du fonctionnement familial. Les relations perturbées avec les parents, ont maintenu dans l'insécurité l'adolescent qui est incapable de s'en séparer, mais elles ont aussi activé une agressivité qui rend insupportable la poursuite du lien avec les parents, que vient conflictualiser encore la sexualisation du corps et des relations. Les désirs œdipiens sont exacerbés par les besoins de dépendance, et sont infiltrés d'agressivité. La poursuite de la dépendance, à laquelle l'adolescent se sent incapable d'échapper, représente une menace de destruction réciproque. Le geste suicidaire réalise alors une tentative désespérée de se soustraire à une situation vécue comme intolérable, mais il est aussi un ultime effort pour la modifier. Désir de mort et désir de vie coexistent étroitement dans ces comportements irréductiblement contradictoires. Geste dirigé sur le corps, il vise en fait l'atténuation des conflits psychiques et familiaux, tout en restant un mode de solution pathologique, inefficace et néfaste. Volonté de changement, il comporte aussi le risque de figer l'adolescent et sa famille dans un fonctionnement morbide : le geste suicidaire réalise, à la fois, une attaque du corps sexué et une attaque des parents, une intrusion meurtrière dans l'espace psychique parental. L'adolescent qui a tenté de se suicider, a tué une partie de lui-même qui est projetée dans le psychisme parental. L'existence de cette partie morte, dans l'appareil psychique familial, y pérennise le processus mortifère. Les défis de la thérapeutique des tentatives de suicide de l'adolescent, consistent à réussir à réduire la dynamique suicidaire, dans les familles dont la coopération est limitée par l'intensité de l'hostilité ouverte, ou par le déni de la destructivité cachée.

In 1994, the diagnosis of transsexualism was replaced with ”gender identity disorder" by the DSM-IV committee. Transsexualism is associated with distress because the individual, due to his/her physical characteristics, is not treated as a representative of the gender he/she identifies with and cannot therefore live in a gender role with which he/she identifies. The sexual anatomy of the body also feels alien and inappropriate. Transsexualism that causes distress is treated with sex reassignment therapy. The aim is to alter body characteristics and anatomy closer to those of the target gender as well as to facilitate the integration into the social role of the opposite sex. Investigations and surgery are often only available in specialist centres.
