Book excerpt: L'objectif de cette étude est de prouver l'efficacité de la gouttière dentaire à rampe (GDR) dans le traitement précoce des classes II. La GDR est un activateur en PVC souple médical associé à une force extra-orale avec appui occipital. Une étude rétrospective sur 30 patients a été réalisée et comparée à un groupe témoin de 30 sujets. Les analyses céphalométriques conventionnelles, les superpositions structurales de Lautrou ainsi que l'analyse LDV ont été réalisée sur les téléradiographies de profil des 60 cas et ont permis leur comparaison. La GDR provoque une modification significative de la quantité et de la direction de la croissance mandibulaire mis en évidence par une allongement de la diagonale mandibulaire et une modification des rapports dento-alvéolaires aux deux arcades. La GDR aide à la correction des classes II par une avancée symphysaire dans la face. La GDR créé de nouveaux rapports dento-labio-lingaux et favorise une croissance harmonieuse de la sphère oro-faciale. Le contrôle des principaux effets parasites des activateurs est lié à l'utilisation de la FEO à traction haute. La GDR s'intègre donc parfaitement dans un traitement en deux phases des classes II. La phase multiattache qui s'ensuit se trouve significativement simplifiée et raccourcie

Cette dernière faisant, alors appel à toute la préparation orthodontique pré-opératoire. Il est nécessaire de revenir sur les techniques chirurgicales employées dans ce type de traitement. De plus, il est important pour l’orthodontiste de mettre en avant l’importance d’un bilan détaillé de l’étiologie des récidives, afin d’en prévenir l’apparition. En effet, l’élaboration d’un diagnostic initial spécifique, pourrait permettre de distinguer les patients dits à risque de récidive et pour le praticien agir de façon prévisible en aval et en amont de la chirurgie. Nous tenterons donc de mettre en évidence les moyens de prévention afin de proposer plus sereinement un traitement chirurgical aux patients le nécessitant, et de façon stable. Nous nous intéresserons aux autres perspectives de traitements chirurgicaux tels que la distraction osseuse ou la chirurgie assistée par ordinateur. Puis nous aborderons le point de vu légal de la récidive.
