Chez la femme, il est l'affection tumorale la plus fréquente et la première cause de décès par cancer. Selon l'Institut National du Cancer 24 % des cancers du sein touche les femmes âgées de plus de 74 ans. Le dépistage organisé du cancer du sein concerne les femmes de 50 à 74 ans. Après cet âge, le dépistage repose en parti sur le médecin traitant premier recours pour tous les âges et qui est chargé de déterminer la modalité de surveillance la plus adaptée. La palpation mammaire est recommandée par la Haute Autorité de Santé annuellement en France à partir de 25 ans. L'objectif était de recueillir l'avis des médecins généralistes sur la palpation mammaire chez les femmes de plus de 74 ans et sur les facteurs prédictifs au dépistage du cancer du sein dans cette population. Il s'agit d'une enquête d'opinion, descriptive, transversale, anonyme. Un questionnaire a été réalisé et envoyé par voie électronique le 27 février 2020 à 4680 médecins généralistes de la région Auvergne Rhône-Alpes par l'URPS AuRa. 246 des 4680 médecins sollicités ont répondu (taux de réponse 5,2%) du 27 février au 15 mars 2020. Le dépistage par mammographie au-delà de 74 ans est discuté en raison d'une balance incertaine entre bénéfices et risques. La décision de la poursuite du dépistage de la part des médecins doit prendre en compte l'âge, les comorbidités et l'espérance de vie. Le point de vue des patientes sur cette démarche a été peu étudié. Notre étude a pour objectif d'étudier les facteurs associés à la poursuite du dépistage du cancer du sein par mammographie auprès des femmes de plus de 74 ans. Nous avons réalisé une étude observationnelle descriptive multicentrique avec l'aide d'un questionnaire de 19 items distribué à des femmes de plus de 74 ans en milieu médical ainsi qu'en dehors. Nous avons pu analyser 144 réponses. Les facteurs associés significativement à la poursuite du dépistage du cancer du sein étaient : l'état de santé perçu, l'autonomie vis-à-vis des actes de la vie quotidienne, l'antécédent de cancer du sein chez un proche, la réalisation de la palpation mammaire par un professionnel de santé, la participation au dépistage organisé et enfin l'absence de difficultés à se rendre au centre de radiologie. Les connaissances sur le cancer du sein ou la mammographie n'étaient pas associées à une conduite de dépistage. La majorité des femmes interrogées étaient favorables à la poursuite du dépistage du cancer du sein. Cette étude met en lumière les motivations des femmes à vouloir poursuivre le dépistage au-delà de 74 ans. Souvent placé au cœur de la décision concernant le dépistage, c'est au médecin généraliste d'informer et d'accompagner les femmes dans le choix de poursuivre ou non le dépistage.

Même si la HAS recommande un examen clinique des seins par un professionnel pour toutes les femmes dès l'âge de 25 ans, le dépistage organisé concerne seulement les femmes âgées de 50 à 74 ans. Pourtant beaucoup de femmes de moins de 50 ans ont déjà eu une imagerie mammaire. L'objectif de notre étude a donc été d'identifier ces femmes, leur profil, pourquoi elles avaient réalisé cette imagerie et les conséquences de celle-ci. Matériel et méthode : Il s'agit d'une étude quantitative, observationnelle, descriptive, rétrospective et multicentrique. Des questionnaires ont été récoltés anonymement auprès de 167 femmes âgées de 25 à 49 ayant passé une imagerie mammaire. RÉSULTATS : la grande majorité des patientes (60.4%) sont âgées de plus de 40 ans, ont eu à la fois une mammographie et une échographie mammaire (51.5%) et sont suivies par un(e) gynécologue (77.6%), ce qui est un peu moins vrai chez les patientes plus jeunes, de moins de 40 ans qui sont un peu plus suivies par un(e) médecin généraliste. La raison la plus évoquée est la présence d'antécédents familiaux de cancer du sein. Ces imageries ne sont pas anodines et engendrent du stress (pour 68.6% des patientes) et sont douloureuses (pour la mammographie (54.1%), et pourtant elles sont souvent sans suite pour presque la moitié des patientes (47%). Conclusion : parmi ces imageries, beaucoup sont réalisées après 40 ans et souvent dans le cadre d'un dépistage systématique. Les recommandations actuelles de dépistage organisé sont certainement amenées à être modifiées prochainement. D'autre part, plus de la moitié des patientes n'ont pas d'examen clinique annuel des seins comme recommandé par la HAS. Est-ce également amené à changer ?
