Compte tenu de la rareté des études sur l’impact de cette maladie sur la qualité de vie (QdV) et la réinsertion à long terme, une étude a été réalisée en population générale à partir de registres des cancers. L’objectif était d’étudier la QdV et la réinsertion 5, 10 et 15 ans après le diagnostic de patientes en longue rémission d’un cancer du sein (cas) par comparaison à des témoins. Les cas ont été tirés au sort à partir de trois registres des cancers et les témoins ont été tirés au sort sur les listes électorales. Cinq questionnaires ont été utilisés : QLQ-C30, SF-36, MFI-20, STAI et un questionnaire de conditions de vie. Six cent cinquante-deux cas et 1 188 témoins ont participé à l’étude. Une différence cliniquement significative a été mise en évidence entre les cas toujours en vie 5 ans après le diagnostic et les témoins pour les dimensions suivantes : activité physique, limitations dues à l’état physique, limitations dues à l’état psychique et fatigue. Ces différences s’estompaient avec le temps pour pratiquement tous les indicateurs. Les résultats sur les conditions de vie ont montré que sur certains points, les cas présentent une évolution positive depuis leur diagnostic par rapport aux témoins, notamment dans leur vie familiale et sociale. Cependant, des difficultés persistent dans leur vie sexuelle et professionnelle et lors d’une demande de prêt financier. Notre étude a permis de montrer que la QdV et les conditions de vie des patientes en longue rémission sont, en général, différentes de celles des témoins de population.

Book excerpt: Cette recherche a pour objectifs de décrire et surtout d'expliquer de décrire et surtout d'expliquer la qualité de vie et le développement post-traumatique (changements positifs psychologiques vécus suite à un événement de vie difficile) de femmes étant actuellemnt de 5 à 15 ans du diagnostic d'un cancer du sein, en comparaison avec d'autres n'ayant pas eu le acncer et faisant référence à l'événement le plus difficile vécu dans les 15 dernières années de leur vie. Méthode : cette étude transversale a été menée par questionnaires auprès de 307 femmes ayant eu le cancer et 132 femmes constituant une position contrôle. Des entretiens semi-directifs, conduits auprès de 28 femmes anciennement cancéreuses ont complété les questionnaires dans l'appréhension du développement post-traumatiques après le cancer. Résultats : La qualité de vie des femmes à long terme du cancer est satisfaisante et comparable à celle de la population contrôle. Bien que des changements positifs soient effectivement déclarés, ils ne semblent pas spécifiques au cancer. Dans les deux populations, la qualité de vie mentale dépend essentiellement de la personnalité et du soutien perçu, la qualité de vie physique de variables sociodémographiques, médicales et des séquelles laissées par l'événement, le développement post-traumatique du coping. Conclusion : dans l'ensemble et à long terme, le cancer ne paraît pas plus délétère que d'autres événements difficiles sur la qualité de vie. Il convient néanmoins de ne pas minimiser l'impact d'un cancer du sein dans la vie d'une femme.

Book excerpt: Introduction : en 2018, 12146 décès par cancer du sein ont été recensé en France. C'est le cancer le plus fréquent et le plus meurtrier chez la femme. Dès l'annonce de la pathologie, au cours des traitements et pendant la rémission, la santé globale et la qualité de vie des femmes sont considérablement altérées. L'objectif principal de ce travail était d'évaluer l'impact du programme d'activité physique proposé à l'hôpital de Cahors sur la qualité de vie de femmes traitées pour un cancer du sein. Matériel et méthode : nous avons mené une étude qualitative s'inspirant de l'analyse phénoménologique interprétative en suivant les critères COREQ. Le recueil des données a été réalisé par le biais d'entretiens individuels semi-dirigés. Résultats : 12 entretiens réalisés entre juin 2021 et juillet 2022 ont été analysés. Notre analyse phénoménologique et interprétative a fait émerger trois thèmes centraux qui décrivent l'expérience des femmes traitées pour un cancer du sein ayant participé à une activité physique adaptée. Ces thèmes s'articulent autour de la reconquête corporelle et identitaire, de la réinsertion sociale et du programme comme fil conducteur de la vie. Discussion : le programme semble avoir été une aide importante dans la reconstruction de soi, dans la reconstruction de leur féminité et dans la reprise du contrôle de leur vie. Il a permis la réappropriation des rôles sociaux et la levée de l'isolement sociale. Il s'inscrit dans une volonté de prise en charge globale et d'amélioration de la qualité de vie. Conclusion : L'intégration d'un protocole d'activité physique dans le parcours de soins des femmes atteintes de cancer du sein paraît être une option viable pour le suivi dans l'après cancer avec un encadrement rapproché, une diminution des effets indésirables à long terme et une amélioration de globale de la qualité de vie.

Its subject, reflected in the title "Sciences and Empires", is not a currently-accepted sub-discipline of the history of science; rather, it refers to a set of questions which found autonomy only recently. The terminology was strongly debated by the participants and, as is frequently suggested in this book, awaits fuller clarification.

Book excerpt: This important theoretical work by Paul de Man sets forth a mode of reading and interpretation based on exemplary texts by Rousseau, Nietzsche, Rilke, and Proust. The readings start from unresolved difficulties in the critical traditions engendered by these authors, and they return to the places in the text where those difficulties are most apparent or most incisively reflected upon. The close reading leads to the elaboration of a more general model of textual understanding, in which de Man shows that the thematic aspects of the texts--their assertions of truth or falsehood as well as their assertions of values--are linked to specific modes of figuration that can be identified and described. The description of synchronic figures of substitution leads, by an inner logic embedded in the structure of all tropes, to extended, narrative figures or allegories. De Man poses the question whether such self-generating systems of figuration can account fully for the intricacies of meaning and of signification they produce. Throughout the book, issues in contemporary criticism are addressed analytically rather than polemically. Traditional oppositions are put in question by a rhetorical analysis which demonstrates why literary texts are such powerful sources of meaning yet epistemologically so unreliable. Since the structure which underlies this tension belongs to language in general and is not confined to literary texts, the book, starting out as practical and historical criticism or as the demonstration of a theory of literary reading, leads into larger questions pertaining to the philosophy of language. Completely new are chapters describing the international system by which morphine and other opioids are made available for medical purposes. The book has two parts. Part one provides a practical guide to the relief of cancer pain, concentrating on drug treatment as the mainstay of pain management. The most extensive section sets out detailed guidelines for the selection and prescribing of non-opioid analgesics, opioid analgesics, drugs for neuropathic pain, and adjuvant drugs for the treatment of adverse effects, the enhancement of pain relief and the management of concomitant psychological disturbances. Information ranges from explanations of how specific drugs work, through the precautions to take in the presence of certain disorders, to a list of factors that influence the effectiveness of opioids. Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain. Part two provides a guide to opioid availability. A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.
