Une amélioration significative des performances auditives est observée après implantation cochléaire (IC), entraînant ainsi une amélioration dans le domaine de la qualité de vie (QdV). L'objectif de l'étude est de déterminer l'apport de l'IC dans ce domaine, en utilisant les outils disponibles pour son évaluation. La musique est également un facteur important et sous-évalué de la QdV. Notre deuxième objectif est d'étudier l'écoute musicale des patients, et l'aide apportée par l'IC dans cette dernière. Matériel et méthodes. Il s'agit d'une étude rétrospective sur 30 patients qui ont rempli deux questionnaires de QdV, avant et après implantation, le Nijmegen Cochlear Implant Questionnaire (NCIQ) spécifique à l'implant cochléaire, et le Health Utilities Index mark 3 (HUI 3), questionnaire général de qualité de vie, et un questionnaire portant sur leur pratique musicale, entre 6 mois et 5 ans après IC. Résultats. Une amélioration des scores totaux du questionnaire HUI3 par l'implantation a été retrouvée pour 28 patients (93.33%) en particulier dans les sous domaines de l'audition et émotionnels, avec un gain moyen de 1.66 point et de 1.17 point. Une amélioration a été retrouvée dans tous les sous domaines du NCIQ, y compris chez les patients implantés entre 6 mois et un an; la perception des sons simples bénéficiant du meilleur gain (+270 points). Dans le domaine musical, 10 patients (40%) n'ont pu reprendre leur écoute musicale de manière satisfaisante. Conclusion. L'IC permet chez l'adulte présentant une déficience auditive une réhabilitation sur ce plan notamment en termes de discrimination de la parole. Dans un délai plus court il permet l'amélioration de la QdV en minimisant les répercussions de la surdité dans les activités quotidiennes. Une rééducation musicale personnalisée, dans les suites de l'implantation, trouve sa place au cours de la prise en charge orthophonique aux côtés des moyens de réhabilitation habituels.

C’est pourquoi, cette étude, en s’appuyant sur les données d’une thèse présentée en 2002, a pour objectif de l’évaluer. En ré-interrogeant les mêmes patients questionnés en 2002, nous avons pu comparer leurs résultats en 2002 et en 2008. Deux questionnaires ont été utilisés : le Nijmegen, auto-questionnaire spécifique à l’implantation cochléaire et le SF-36, questionnaire généraliste de qualité de vie. Un groupe de patients en attente d’implantation a également été interrogé afin de pouvoir les comparer au groupe de patients implantés. Les résultats des 68 patients implantés cochléaires ont révélé une stabilisation des résultats plus de 6 ans après l’implantation. La comparaison du groupe en attente d’implant avec le groupe de patients implantés souligne une nette amélioration de la qualité de vie après l’implantation cochléaire.
